摘要 目的:探讨脉冲染料激光(pulsed dye laser,PDL)不同治疗周期对兔耳创面上皮细胞向间质细胞转化的影响及该现象对瘢痕增生的作用。方法:新西兰大耳白兔12只建立兔耳腹侧增生性瘢痕模型,应用595nm Vbeam脉冲染料激光分别采用一周、二周、三周的治疗周期进行瘢痕的激光干预,在治疗前及治疗2、4、6、8周测量瘢痕厚度,治疗结束后取材行组织形态学观察,免疫组织化学方法测定各组上皮间充质转化(Epithelial-mesenchymal Transformation,EMT)正相关蛋白及负相关蛋白的表达情况。结果:兔耳各组模型均有瘢痕增生,EMT表面标记物表达阳性;PDL治疗结束后,一周治疗组及二周治疗组均表现出TGF-β1、Vim表达下调,Ecad表达上调,产生明显的瘢痕抑制效应,与对照组相比有显著差异;三周治疗组与对照组、一周治疗组和二周治疗组相比无明显差异。结论:PDL可能通过干预EMT的表达产生瘢痕抑制作用;治疗周期为两周较为适宜。 Objective: investigate the influence of PDL in different treatment cycles on the EMT of rabbit ear scar and the effect on hyperplasia.Methods: 12 New Zealand big-ear rabbits were used to establish the hypertrophic scar in ventral rabbit ears.595 nm pulsed dye laser was used to treat the scar for 1 week,2 weeks and 3 weeks respectively.The thickness of scar was measured before treatment and at 2,4,6,8 weeks,sample and observe the histomorphology after treatment.Immunohistochemical was applied to detect the expression of positive and negative related proteins in epithelial mesenchymal transformation.Results: all the models of rabbit ears had hypertrophic scars and EMT markers were positive.After irradiation with PDL,1 week treatment group and 2 weeks treatment group showed down regulation of TGF-β1 and Vimentin expression,up regulation of E-cadherin expression,and had obvious scar inhibitory effect.There was a significant difference compared with the control group.While there was no significant difference of the 3 weeks treatment group compared with the control group,the 1 week treatment group and the 2 weeks treatment group.Conclusion: PDL may inhibit scar formation through interfering the expression of EMT,and the proper treatment period is two weeks.
