基于云平台的远程监测及综合管理对门诊高血压患者血压达标率的影响

Effects of cloud-based telemonitoring and integrated management on blood pressure goal achievement among outpatients with hypertension

目的探讨基于云平台的远程监测及综合管理对门诊高血压患者血压达标率的影响。方法选取我院2016年6-10月门诊高血压患者172例,随机分为观察组(84例)和对照组(88例),观察组采用基于云平台的远程监测及综合管理模式进行延续管理,对照组沿用传统的门诊随访模式。干预3个月后比较两组24 h、白天、夜间、清晨的平均收缩压(systolic blood pressure,SBP)和舒张压(diatolic blood pressure,DBP)及24 h、白天、夜间、清晨的血压达标率。结果干预3个月后,观察组24 h、白天、夜间、清晨的平均SBP和DBP均有显著的下降,与对照组的各血压值差异有统计学意义(P均<0.05)。观察组在干预后24 h、白天、夜间、清晨血压达标率分别为72.7%、82.1%、59.5%、54.8%,对照组分别为31.8%、38.6%、20.5%、19.3%,两组差异均有统计学意义(P均<0.05)。结论基于云平台的远程监测及综合管理相对于传统门诊随访模式,指导患者血压控制效果更显著,值得推广。

Objective To explore the effect of cloud-based blood pressure telemonitoring and integrated management in outpatients with hypertension. Methods A total of 172 hypertensive patients in our hospital were selected from June to October in 2016 and randomly divided into observation group（n=84） and control group（n=88）. Patients in observation group received cloud-based blood pressure telemonitoring and integrated management, while control group followed the routine outpatient follow-up. Three months later, the average 24 h, daytime, nighttime, morning SBP, DBP and success rate of blood pressure goal achievement were compared between the two groups. Results After 3 months, the average SBP, DBP of 24 h, daytime, nighttime, morning in the observation group decreased significantly when compared with the control group（All P〈0.05）. After intervention, the 24 h, daytime, nighttime, morning success rate of blood pressure goal achievement were 72.7%, 82.1%, 59.5%, 54.8%, respectively in the observation group, and 31.8%, 38.6%, 20.5%, 19.3%, respectively in the control group, with statistically significant difference between two groups（All P〈0.05）. Conclusion Compared with the traditional outpatient follow-up, the effect of cloud-based blood pressure telemonitoring and integrated management are effective on blood pressure control in hyperstensive patients, which is worthy of popularization.
