湿化高流量鼻导管氧疗在急性低氧性呼吸衰竭中的应用价值 被引量:25

摘要 目的:探讨湿化高流量鼻导管(humidified high flow nasal cannula,HHFNC)氧疗在急性低氧性呼吸衰竭中的应用价值。方法:选择某院重症医学科2016年1—6月收治的37例进行氧疗的急性低氧性呼吸衰竭患者,随机分成面罩(face mask,FM)组(n=18)和HHFNC组(n=19)。分别监测治疗前、治疗1 h、治疗5 h血气分析数值变化。结果:2组治疗1 h、治疗5 h时动脉血氧分压[p_a(O_2)]均显著高于治疗前(P<0.05)。治疗1 h、治疗5 h组间数据比较,HHFNC组p_a(O_2)均显著高于FM组(P<0.05)。结论:HHFNC氧疗对急性低氧性呼吸衰竭具有较好的治疗效果,既能有效纠正缺氧,又能提供很好的舒适度。 Objective To explore the clinical value of humidified high flow nasal cannula oxygen therapy(HHFNC) oxygen therapy for acute hypoxic respiratory failure. Methods Totally 37 patients with acute hypoxic respiratory failure were enrolled who entered ICU from January 2016 to June 2016 and randomly divided into FM group(n =18) and HHFNC group(n =19).The values of blood gas analysis were recorded as before treatment, 1 h and 5 h after therapy, respectively. Results 1 h and 5 h after therapy, both groups had significant increase in p_a(O_2) compared to before treatment(P0.05). Meanwhile, HHFNC group had higher level of p_a(O_2) in comparison to FM group(P0.05). Conclusion HHFNC oxygen therapy has a therapeutic effect on acute hypoxic respiratory failure which helps to correct hypoxemia and improve subject feeling of comfort.
