无创呼吸机双向正压通气救治急性左心衰临床疗效

Clinical efficacy of Bi-PAP non-invasive ventilation in patients with acute left heart failure

目的观察无创呼吸机双向正压通气救治急性左心衰的临床疗效。方法将2015年5月至2017年5月于我院CCU救治符合纳入标准的112例急性左心衰患者随机分为试验组和对照组,各56例。试验组给予无创呼吸机双向正压通气辅助呼吸及常规抗心衰药物治疗;对照组给予常规抗心衰药物治疗及乙醇湿化吸氧。对比两组治疗前及治疗后2 h呼吸频率、心率、平均动脉压、血氧饱和度的情况以及治疗效果。结果两组治疗前呼吸频率、心率、平均动脉压、血氧饱和度无明显区别(P>0.05),治疗后2 h试验组的上述指标均明显优于对照组(P<0.05)。试验组的总有效率明显高于对照组,差异有统计学意义(92.86%vs.71.42%,P<0.01)。结论无创呼吸机双向正压通气联合常规抗心衰药物治疗对急性心衰发作具有显著的疗效。

Objective To observe the clinical efficacy of Bi-PAP non-invasive ventilation in patients with acute left heart failure. Methods A total of 112 patients with acute left heart failure were randomly divided into experimental group and control group,each with 56 cases. They had been treated in CCU of our hospital from May 2015 to May 2017. The experimental group was given Bi-PAP non-invasive ventilation and conventional anti-heart-failure drug therapy while the control group was treated with conventional anti-heart-failure drugs and by alcohol-humidification oxygen inhalation. The respiratory rate,heart rate,mean arterial blood pressure,oxygen saturation and curative effect were compared between the two groups before and two hours after the treatment. Results Prior to treatment,the respiratory rate,heart rate,mean arterial blood pressure and oxygen saturation did not vary significantly（ P 〉 0. 05）. After two hours of treatment,these indexes in the experimental group were all significantly higher than those in the control group（ P 〈 0. 05）. The total effective rate in the experimental group was significantly higher than that in the control group（ 92. 86% vs. 71. 42%）,with statistically significant difference（ P 〈 0. 01）. Conclusion A combination of Bi-PAP non-invasive ventilation and conventional anti-heart-failure drug therapy proves to be significantly effective for patients with acute heart failure.
