痛经的精神心理因素探讨

Study of the psychological changes in dysmenorrhea

目的:调查痛经患者及无痛经女性的心理健康状态,探讨痛经对心理因素的影响.方法:回顾性分析月经周期均规律的201例痛经患者及63例无痛经女性的临床资料,对比分析其在月经周期3个时间段(月经期前、月经期、月经期后)的心理健康状态.结果:痛经女性组90项证状自评量表(SCL-90)中躯体化、抑郁、焦虑、精神病性、总分及总均分与对照组相比较,在月经周期的不同时段均显著高于对照组,有统计学差异(P<0.05);痛经的疼痛程度与敌对症状、躯体化、抑郁、焦虑、恐怖具有相关性(P<0.05).结论:痛经的疼痛程度可能对患者的精神心理造成一定影响,在对痛经患者实施治疗方案同时应重视心理疏导治疗.

Aim： To investigate the mental health states of normal people and patients with dysmenorrhea and to explore the influence of dysmenorrhea on psychological aspects. Methods： To study the psychological health states of the research subjects in three periods,pre-menstrual,menstrual and postmenstrual period during menstrual cycles, a retrospective analysis was conducted in 201 dysmenorrheal patients and 63 normal people collected from the gynaecological clinic and inpatient department of Guangdong Province Traditional Chinese Medical Hospital from February 2014 to February2015. Results： Somatization,depression,anxiety,psychotism,total marks and mean marks of SCL-90 in the cases at any time of the menstrual cycle were significantly higher than those in the control（ P 0. 05）. There is a positive correlation between the pain degree of dysmenorrhea and hostile,somatization,depression, anxiety, psychotism and terror（ P 0. 05）. Conclusion： The pain of dysmenorrhea may affect the mental states of the patients. In the treatment of dysmenorrhea attentions should be paid to the psychological changes.
