动态心电图(Holter)评价起搏器功能状态的价值分析

Value analysis of dynamic electrocardiogram( Holter) in evaluating pacemaker function

目的:观察心脏起搏器植入术后患者采用动态心电图(Holter)评价起搏器功能状态的价值。方法:选取已安装心脏起搏器的161例患者作为研究对象,同时行动态心电图(Holter)检查及常规12导联心电图检查,比较两者的检查结果并进行分析。结果:行Holter后得知心房起搏异常2例、感知异常8例,心室起搏异常3例、感知异常2例,起搏器特殊功能132例,检出率为91.30%;而常规12导联心电图仅发现心房起搏异常1例,心房感知异常2例,起搏器特殊功能5例,检出率仅为4.97%,两组相比差异具有统计学意义(P<0.001)。并且根据Horter发现的异常对起搏器参数进行适当的调整,取得良好效果。结论:Holter能更准确、更完整地提供心脏起搏器的功能状态信息,且较常规12导联心电图检出率更高,是起搏器植入术后患者随访的重要手段,值得在临床上普及和推广。

Objective To observe the value of dynamic electrocardiogram（ Holter） in evaluating the functional status of pacemakers after pacemaker implantation. Method 161 cases of pacemaker patients were selected as the research object,and dynamic electrocardiogram（ Holter） examination and 12 lead ECG examination,compare the examination results and analysis. Results The heart atrial pacing after Holter was abnormal in 2 cases,8 cases of abnormal perception; 3 cases of abnormal ventricular pacing and sensing abnormalities in 2 cases,132 cases of pacemaker function,detection rate（ 91. 30%）,while the conventional 12 lead ECG found only 1 cases of abnormal atrial pacing,atrial sensing abnormalities in 2 cases,5 cases of pacemaker special function,detection rate was only two（ 4. 97%）,compared with statistical significance（ P〈 0. 001）. And according to the abnormalities found by Holter,the pacemaker parameters were adjusted properly,and good results were achieved. Conclusion The function of state information Holter can provide more accurate and more complete cardiac pacemaker,and compared with the conventional 12 lead ECG detection rate is higher,is an important means of patient follow-up after pacemaker implantation,so it is worth popularizing in clinic.
