呼出气一氧化氮对支气管哮喘诊疗价值的研究

Study on the value of exhaled nitric oxide in the diagnosis and treatment of bronchial asthma

目的:探讨呼出气一氧化氮(FeNO)指标监测对于哮喘诊断和治疗评估的参考价值。方法:选取河南大学第一附属医院呼吸内一科2016年3月至2017年2月收治的69例有喘息、气急、胸闷或咳嗽等临床症状疑似支气管哮喘的患者作为研究对象,均行支气管舒张试验及FeNO监测,进行诊断及治疗评估。结果:相比于支气管舒张试验阴性患者,支气管舒张试验阳性患者的各项肺功能指标水平相对较低,而FeNO监测指标相对较高。在哮喘组患者中,FeNO值显著高于非哮喘组,对比差异具有统计学意义(P<0.05)。经治疗,观察组患者的各项肺功能指标均得以显著改善,其中以FeNO的降幅最为明显,治疗前后对比差异显著(P<0.05)。结论:在肺功能检查的基础上,FeNO监测结果的变化,能够准确地反映患者的病情和预后,为支气管哮喘的诊断和治疗评估提供了有价值的参考。

Objective：To investigate the reference value of exhaled nitric oxide（FeNO）monitoring in the diagnosis and treatment of asthma.Methods：Selected 69 cases of patients with suspected clinical symptoms of bronchial asthma,such as wheezing,shortness of breath,chest tightness or cough treated in Respiratory Medicine Dept 1 of The first affiliated hospital of henan university from March 2016 to February 2017 as the research object.They were all performed with bronchial dilation test and FeNO monitoring,according which we can conduct diagnosis and treatment assessment.Results：Compared to the bronchodilation test negative patients,the pulmonary function index level of the bronchodilation test positive patients is relatively low,while the FeNO index is relatively high.In the asthma group,value of FeNO monitoring was significantly higher than the nonasthma group,the difference was statistically significant（P〈0.05）.After treatment,the lung function indexes of the observation group were significantly improved,among which the decrease of FeNO was the most obvious,and the difference was significant（P〈0.05）before and after treatment.Conclusion：Based on the pulmonary function test,refering to the changes of FeNO monitoring results,it can accurately reflect the patient＇s condition and prognosis,and provide a valuable reference for the diagnosis and treatment evaluation of bronchial asthma.
