良性阵发性位置性眩晕126例临床分析

Benign Paroxysmal Positional Vertigo:An Analysis of 126 Cases

目的统计分析我院良性阵发性位置性眩晕患者的临床资料,以指导临床诊断及治疗。方法以最新版诊断标准为依据,筛选良性阵发性位置性眩晕患者入组,统计分析其年龄、性别、发病诱因、一般临床表现、眼震电图检查结果及手法复位的疗效。结果共入组126例患者,年龄为31~81岁,平均年龄为(53.7±7.6)岁,男∶女=1∶1.2;77例(61.1%)患者起病前有明确的剧烈的头位或体位变化,26例(20.6%)患者出现剧烈呕吐;按部位分类,后半规管90例(71.4%),上半规管10例(8.0%),水平半规管18例(14.3%),混合型8例(6.3%),管结石103例(81.7%),嵴帽结石23例(18.3%);91例(72.2%)经过手法复位后治愈。结论良性阵发性位置性眩晕发病人群以中老年为主,女性多于男性,后半规管发病率最高,手法复位有助于患者缓解症状。

Objective To analyze the clinical data of patients with benign paroxysmal positional vertigo （BPPV） to guide clinical diagnosis and treatment. Methods Based on the latest diagnosis criterion, patients with BPPV were enrolled. Age, gender, predisposing factors, general clinical manifestations, ENG and curative effect of manual reduction were analyzed. Results 126 cases with BPPV were enrolled, aged 31-81 years old, mean （53.7±7.6） years old, male：female=1 ：1.2; 77 cases （61.1%） had definite sharp head position or postural changes before BPPV onset; 26 cases （20.6%） had severe vomiting. All cases were classified by location： posterior semicircular canal BPPV in 90 cases （71.4%）, superior semicircular canal BPPV in 10 cases （8.0%）, horizontal semicircular canal BPPV in 18 cases （14.3%）, mixed types in 8 cases （6.3%）; 103 cases （81.7%） were semicircular canal calculi, and 23 cases （18.3%） were cupula calculi. 91 cases （72.2%） underwent manual therapy were cured. Conclusion The main diseased population of BPPV is the middle-aged and elderly, and there are more female patients than male patients. The incidence of posterior semicircular canal BPPV is the highest. Manual reduction Can help patients to relieve the symptoms.
