标准外伤大骨瓣减压术治疗重型颅脑损伤的临床效果及预后观察

Clinical effect and prognosis of standard large trauma craniotomy in the treatment of severe craniocerebral injury

目的探讨标准外伤大骨瓣减压术治疗重型颅脑损伤的临床效果。方法将200例重型颅脑损伤患者随机分为两组,各100例。观察组采用标准外伤大骨瓣减压术治疗,对照组采用常规去骨瓣减压术治疗,比较两组疗效、预后及并发症发生情况。结果两组患者术后1 d、3 d、7 d同期颅内压比较,观察组均低于对照组(P<0.01);术后随访1年,观察组预后GOS评分等级优于对照组(P<0.01);观察组术后并发症发生率低于对照组(P<0.01)。结论标准外伤大骨瓣减压术治疗重型颅脑损伤效果显著,可有效降低颅内压,术后并发症少,改善预后。

Objective To investigate the clinical effect of standard large trauma craniotomy in the treatment of severe craniocerebral injury. Methods Two hundred cases of severe craniocerebral injury patients were randomly divided into 2 groups,each has100 cases. Observation group adopted standard large trauma craniotomy,while the control group adopted routine decompressive craniectomy treatment method and prognosis complications were compared between the two groups. Results The intracranial pressure of observation group were lower than those of control group at 1 d,3 d,7 d after operative （P 〈0. 01）; after one year＇s follow-up,the GOS of the observation group was better than the control group （P 〈0. 01）. Conclusion Standard large trauma craniotomy is effective in the treatment of severe traumatic brain injury. It can effectively reduce intracranial pressure,reduce postoperative complications and improve prognosis.
