自拟清热祛瘀益肾汤联合环磷酰胺治疗早期狼疮性肾炎疗效观察

Observation of the curative effect of Qingre Quyu Yishen decoction combined with cyclophosphamide on early early lupus nephritis

目的观察自拟清热祛瘀益肾汤联合环磷酰胺治疗早期狼疮性肾炎的临床疗效。方法将100例早期狼疮性肾炎患者随机分为对照组与观察组,每组50例。对照组给予环磷酰胺静脉滴注,观察组在对照组治疗基础上给予自拟清热祛瘀益肾汤口服,观察2组临床疗效及治疗前后肾功能指标水平、系统性红斑狼疮疾病活动度(SLEDAI)评分、中医证候评分变化情况,统计2组不良反应发生情况。结果治疗1个疗程后,观察组临床总有效率显著高于对照组(P<0.05);观察组治疗后的24 h尿蛋白定量、血清肌酐、血清尿素氮、血尿酸水平、SLEDAI评分、中医证候评分均显著低于对照组;2组治疗期间均未出现明显不良反应。结论自拟清热祛瘀益肾汤联合环磷酰胺治疗早期狼疮性肾炎临床疗效确切,可显著改善肾功能与中医症状,且安全可靠。

Objective It is to observe the clinical effect of Qingre Quyu Yishen decoction combined with cyclophosphamide on early early lupus nephritis. Methods 100 patients with early lupus nephritis were randomly divided into control group and observation group, with 50 cases in each group. The control group was given cyclophosphamide by intravenous drip, the observation group was given Qingre Quyu Yishen decoction orally on the basis of therapy in the control group. The changes of clinical curative effect and the levels of renal functional indexes, systemic lupus erythematosus disease activity of indexes and TCM syndrome scores before and after treatment were observed and the occurrences of side effects were recorded in the 2 groups. Results After one course of treatment, the total clinical effective rate in the observation group was significantly higher than that in the control group （P 〈 0.05）. In the observation group, 24h urinary protein quantification, serum creatinine, serum urea nitrogen, serum uric acid level, SLEDAI scores and TCM syndrome scores were significantly lower than those in the control group. There was no obvious adverse reaction occurred during therapy. Conclusion Qingre Quyu Yishen decoction combined with cyclophosphamide can significantly improve renal function and TCM symptoms, and is safe and reliable in the treatment of early lupus nephritis.
