中医康复护理在急性心肌梗死冠脉支架术后的运用探讨

The Clinical Application of TCM Rehabilitation Nursing in Patients with Acute Myocardial Infarction after Coronary Stent Implantation

目的:探讨中医康复护理在急性心肌梗死冠脉支架术后的作用。方法:选取80例急性心肌梗死冠脉支架术后患者,根据患者入院顺序随机分为对照组及观察组。两组均予常规西医三阶段康复护理,观察组结合中医康复护理。比较两组心肺功能、运动耐力、生存质量。结果:观察组6 min步行试验距离、代谢当量、左室射血分数在PCI术后第4周、第12周均大于同期对照组(P<0.01);对照组6 min步行试验距离、代谢当量、左室射血分数在PCI术后第4周、第12周组内比较,差异无统计学意义(P>0.05);观察组PCI术后第12周6 min步行试验距离、代谢当量、左室射血分数均高于同组第4周(P<0.01)。PCI术后第4周,观察组生存质量各维度评分均高于同期对照组,但组间差异无统计学意义(P>0.05);PCI术后第12周,观察组PF、RP、BP、VT、MH、GH评分高于同组PCI术后第4周(P<0.05),并且观察组PF、RP、BP、VT、MH、GH评分高于同期对照组(P<0.05)。结论:急性心肌梗死冠脉支架术后患者结合中医康复护理,能有效改善患者心肺功能,提高运动耐力,对改善患者生存质量具有积极意义。

Objective： To explore the effect of traditional Chinese medical （TCM） rehabilitation nursing in patients with acute myocardial infarction after coronary stent implantation. Methods： The 80 patients with acute myocardial infarction after coronary stent implantation are randomly divided into control group and study group. The two groups are given conventional western medicine rehabilitation nursing in three stages, study group combined with TCM rehabilitation nursing. The cardiopulmonary function, exercise tolerance and quality of life were compared between the two groups. Results： The study group showed higher six minute walk test distance, metabolic equivalent, left ventricular ejection fraction than the control group in the same period （P〈0.01） at the 4th week and 12th week in PCI; The change of six minute walk test distance, metabolic equivalent, left ventricular ejection fraction the control group are not statistically significant at the 4th week and 12th week in PCI （P〉 0.05）; Compared with 4th week, the study group showed higher six minute walk test distance, metabolic equivalent, left ventricular ejection fraction at 12th week in PCI （P〈0.01）. At 4th week after PCI, the study group showed higher dimension of quality of life scores than the control group, but there was no significant difference （P〉0.05）; Compared with 4th week, the study group showed higher PF, RP, BP, VT, MH, GH score at 12th week after PCI （P〈0.05）, and the study group showed higher PF, RP, BP, VT, MH and GH scores than the control group （P〈0.05）. Conclusion： TCM rehabilitation nursing can effectively improve the patient＇s heart and lung function, improve exercise endurance and increase the quality of life of patients has important significance, in patients with acute myocardial infarction after coronary stent implantation.
