改良早期预警评分在急诊内科中的应用效果分析

Analysis of application effect of modified early warning score in emergency internal medicine

目的探讨改良早期预警评分(MEWS)在急诊内科的应用效果。方法选择2015年6月~2017年6月在我院急诊内科就诊的200例患者为研究对象。所有患者行MEWS评分,比较不同评分患者去向及转归。结果 200例患者128例评分≤4分,72例≥5分。得分≥5分患者ICU入住率及死亡率显著高于≤4分的患者,差异有统计学意义(P<0.05)。7~8分、≥9分患者入住ICU率显著高于5~6分、3~4分以及0~2分患者,5~6分患者显著高于3~4分、0~2分患者,差异有统计学意义(P<0.05)。7~8分以及≥9分患者死亡率显著高于0~2分、3~4分以及5~6分患者,差异有统计学意义(P<0.05)。MEWS得分与ICU入住率成显著正相关的关系(r=0.968)。结论急诊科应用MEWS对评价患者病情具有重要的临床意义,能有效预测患者的转归。

Objective To discuss the application effect of modified early warning score in emergency internal medicine. Methods 200 cases in internal medicine emergency in our hospital from June 2015 to July 2017 were selected as study objects.All cases were evaluated by MEWS.The where abouts and prognosis of patients with different scores in different score group was compared.Results 128 cases scored ≤4 points,and 72 cases scored ≥5 points and were sent to ICU rates,and mortality of cases with MEWS ≥ 5 points were higher than those of those of cases with MEWS ≤4 points, which showed significant difference （P〈0.05）.ICU rates in cases with MEWS 7-8 points,≥ 9 points were higher than those of cases with MEWS 5-6 points, 3-4 points and 0-2 points,and cases with MEWS 5-6 points was higher than that of cases with MEWS 3-4 points and 0-2 points,which showed significant difference （P〈0.05）.Mortality of cases with MEWS 7-8 points and ≥ 9 points were higher than that of the cases with 0-2,3-4 points and 5-6 points,which showed significant difference （P〈0.05）.Conclusion The application of MEWS in emergency department has important clinical significance for evaluating the patients＇ condition,and can effectively predict the prognosis of the patient.
