子宫肌瘤患者腹腔镜下全子宫切除术与开腹手术的临床疗效比较研究

Comparative study of laparoscopic total hysterectomy and laparotomy in patients with uterine fibroids

目的探讨腹腔镜下全子宫切除术与开腹手术应用于子宫肌瘤患者中的临床差异。方法选取2015年5月~2017年4月我院收治的82例子宫肌瘤患者作为研究对象,依照随机数字表法将其分为观察组(n=41)与对照组(n=41)。对照组患者行开腹全子宫切除术治疗,观察组患者行腹腔镜下全子宫切除术治疗。比较两组患者的手术时间、手术出血量、术后排气时间、住院时间、FSH水平、并发症发生率。结果两组患者手术时间比较,差异无统计学意义(P>0.05);观察组患者的手术出血量少于对照组,术后排气时间以及住院时间短于对照组,差异有统计学意义(P<0.05)。在治疗前,两组患者的FSH水平比较,差异无统计学意义(P>0.05);两组患者手术后FSH水平与手术前比较,差异有统计学意义(P>0.05)。对照组患者的并发症发生率远高于观察组,差异有统计学意义(P<0.05)。结论腹腔镜下全子宫切除术相比开腹手术,在临床应用上具有诸多优势,能够在保证正常手术时间的同时,大幅缩短患者手术时间与住院时间,减少手术出血量,改善FSH水平,降低患者并发症发生率,可在临床治疗中大力推广。

Objective To investigate the clinical difference of laparoscopic hysterectomy and laparotomy in patients with uterine fibroids.nethods From May 2015 to April 2017,82 patients with hysteromyoma who treated in our hospital were selected as objects.They were divided into observation group （n=41） and control group （n=41） according to random number table method.The control group was treated with open hysterectomy,and the observation group was treated with laparoscopic total hysterectomy.The operation time,bleeding volume,postoperative exhaust time,hospitalization time,FSH level and complication rate were compared between the two groups.Results Operation time was compared between the two groups,the difference was not statistically significant （P〉0.05）,the patients in the observation group had less blood loss than the control group,the postoperative ventilation time and the hospitalization time were shorter than the control group,the differences were statistically significant （P〈0.05）.Before treatment,the level of FSH was compared between the two groups,the difference was not statistically significant （P〉0.05）.After treatment,the level of FSH of the patients in the observation group were compared with before treatment,the difference was statistically significant （P〈0.05）.In the complication rate,the control group was much higher than the observation group,the difference was statistically significant （P〈0.05）.Conclusion Laparoscopic hysterectomy compared with open surgery,it has many advantages in clinical application,to ensure the normal operation time,shorten surgery time and hospitalization time,reduce the amount of hemorrhage,improve the level of FSH,reduce the incidence of complications,can be popularized in clinical treatment.
