甲钴胺注射液配合芍药甘草汤合桃红四物汤加减治疗糖尿病周围神经病变的可行性分析

Feasibility analysis of Mecobalamin Injection combined with Modified Shaoyaogancao Decoction and Taohongsiwu Decoction in the treatment of diabetic peripheral neuropathy

目的探讨甲钴胺注射液配合芍药甘草汤合桃红四物汤加减治疗糖尿病周围神经病变的临床效果。方法选取2014年2月~2016年12月在我院治疗的80例糖尿病周围神经病变患者,按照随机数字表法分为对照组和研究组,各40例。对照组予以甲钴胺注射液治疗,研究组予以甲钴胺注射液联合芍药甘草汤合桃红四物汤加减治疗。比较两组患者的临床疗效、不良反应发生情况以及干预前后症状积分、运动神经传导速度(MNCV)、感觉神经传导速度(SNCV)、胱抑素C(Cys-C)、同型半胱氨酸(Hcy)水平。结果研究组患者的临床总有效率(92.50%)显著高于对照组(70.00%),差异有统计学意义(P<0.05)。研究组患者干预后的MNCV、SNCV水平显著高于对照组干预后,差异有统计学意义(P<0.05)。研究组干预后的Hcy、Cys-C水平以及各症状积分显著低于对照组,差异有统计学意义(P<0.05)。两组患者均未出现不良反应情况。结论甲钴胺注射液配合芍药甘草汤合桃红四物汤加减治疗糖尿病周围神经病变可显著提高临床效果,改善神经传导速度和症状评分,降低血清Cys-C、Hcy水平。

Objective To investigate the clinical effect of Mecobalamin Injection combined with Modified Shaoyaogancao Decoction and Taohongsiwu Decoction in the treatment of diabetic peripheral neuropathy.Methods 80 patients with diabetic peripheral neuropathy treated in our hospital from February 2014 to December 2016 were selected and divided into the control group and the study group according to the random number table method,40 cases in each group.The control group was treated with Meeobalamin Injection,and the study group was treated with Mecobalamin Injection combined with Modified Shaoyaogancao Decoction and Taohongsiwu Decoction.The clinical curative effect,the incidence of adverse reaction,symptoms score,and the level of motor nerve conduction velocity and （MNCV）,sensory nerve conduction velocity （SNCV）,homocysteine （Hcy）,Cystatin C （Cys-C） levels before and after intervention in the two groups were compared.Results The total effective rate of the patients in the study group （92.50%） was significantly higher than that in the control group （70.00%）,with significant difference （P〈0.05）.The MNCV and SNCV levels of the patients in the study group after intervention were significantly higher than those in the control group,with significant difference （P〈0.05）.The levels of Hcy,Cys-C and symptom scores in the study group after intervention were significantly lower than those in the control group,with significant difference （P〈0.05）.There were no adverse reactions in the two groups.Concluslon Mecohalamin Injection combined with Modified Shaoyaoganeao Decoction and Taohongsiwu Decoction in the treatment of diabetic peripheral neuropathy can significantly improve the clinical efficacy,improve nerve conduction velocity and symptom score,reduce serum Cys-C and Hcy levels.
