4C延续性护理管理模式对脑卒中康复期患者的护理效果研究

Effect of 4C continuous nursing management model on the rehabilitation of stroke patients

目的探讨4C延续性护理管理模式对脑卒中康复期患者的护理效果。方法选取2015年9月~2017年3月我院收治的86例脑卒中患者作为研究对象,按照随机数字法将其分为观察组(n=43)与对照组(n=43)。对照组患者给予常规护理方案,观察组患者额外接受4C延续性护理管理模式。比较两组患者护理结束后的日常行为能力评分、遵医行为评分及护理满意度三个指标。结果观察组患者出院第1、2、6个月的日常生活能力评分均明显高于对照组,差异有统计学意义(P<0.05)。护理后,观察组患者的遵医行为评分显著高于对照组,差异有统计学意义(P<0.05)。观察组患者护理满意度(90.1%)显著高于对照组(76.7%),差异有统计学意义(P<0.05)。结论 4C延续性管理模式能大幅改善脑卒中患者的日常生活能力,提高护理的满意度,值得临床推广研究。

Objective To investigate the nursing effect of 4C continuous nursing management mode in patients with stroke rehabilitation.Methods 86 patients with stroke from September 2015 to March 2017 in our hospital were select- ed as objects and divided into the observation group （n=43） and control group （n=43） according to the random number method.The patients in control group were given regular nursing plans.Patients in the observation group were received the additional 4C continue nursing management mode.The evaluation of three index,such as nursing after the patients daily behavior and compliance the medical behavior and nursing satisfaction of two groups were compared.Results The scores of daily living ability of the patients in the observation group were significantly higher than those in the control group at first,second and 6 months after discharge respectively,and the differences were statistically significant （P〈0.05）. The score of compliance in the observation group after nursing was significantly higher than that in the control group, and the difference was statistically significant （P〈0.05）.The satisfaction degree of the patients in the observation group （90.1%） was significantly higher than that in the control group （76.7%）,and the difference was statistically significant （P〈 0.05）.Conclusion The 4C continuity management model can greatly improve ability of stroke patients the daily life and improve the satisfaction of nursing,which is worthy of clinical research in nature.
