黄芪九物汤联合针刺治疗中风后偏瘫患者47例

Huangqi Jiuwu Decoction Combined with Acupuncture in the Treatment of Hemiplegia after Stroke for 47 Cases

目的探究黄芪九物汤加减与苍龟探穴刺法联合治疗对中风后偏瘫患者日常生活能力及神经功能的影响。方法选取郑州市中医院2014年2月—2016年8月中风后偏瘫患者93例,按建档顺序分组,对照组46例予以苍龟探穴刺法治疗,治疗组47例予以黄芪九物汤加减与苍龟探穴刺法联合治疗,两组均持续治疗4周。分析比较两组治疗前及治疗4周后神经功能缺损程度(NIHSS)、日常生活能力(ADL)及生活质量评分,统计比较两组临床效果。结果两组治疗前NIHSS评分、ADL评分对比差异无统计学意义(P>0.05),治疗组治疗4周后NIHSS评分低于对照组,ADL评分高于对照组,差异有统计学意义(P<0.05);经秩和检验,治疗组临床效果优于对照组,差异有统计学意义(P<0.05);两组治疗前睡眠质量、躯体功能、总体健康等生活质量评分对比差异无统计学意义(P>0.05),治疗组治疗4周后生活质量评分与对照组相比较高,差异有统计学意义(P<0.05)。结论黄芪九物汤加减与苍龟探穴刺法联合治疗中风后偏瘫患者效果显著,可显著促进其日常生活能力及神经功能改善,且有助于其生活质量提升。

Objective To investigate the effect of Huangqi Jiuwu decoction combination of Canggui exploring point acupuncture on daily life ability and neurological function in patients with hemiplegia after stroke. Methods From February 2014 to August 2016,93 patients with hemiplegia after stroke in Zhengzhou Hospital of TCM were divided into control group（ n = 46） and treatment group（ n = 47） according to archiving order. The control group was treated with Canggui exploring point acupuncture,and the treatment group was treated with Huangqi Jiuwu decoction combination with Canggui exploring point acupuncture. Both groups were treated for 4 weeks. Neurological deficit（ NIHSS）,daily living ability（ ADL） and quality of life score before and after treatment for 4 weeks were compared,and clinical effect was analyzed as well. Results There was no significant difference in NIHSS score and ADL score between the two groups before treatment（ P〈0. 05）. The NIHSS score was lower in the treatment group than in the control group at 4 weeks after treatment,and the ADL score of the treatment group was higher than that of the control group（ P〈0. 05）. The results of rank and test showed that the clinical effect of the treatment group was better than that of the control group,and the difference was statistically significant（ P〈0. 05）. There was no significant difference in the quality of life scores between the two groups before and after treatment（ P〈0. 05）. The quality of life scores of the treatment group were higher than those of the control group at 4 weeks after treatment（ P〈0. 05）. Conclusion： The combination of Huangqi Jiuwu decoction with Canggui exploring point acupuncture can improve the daily life ability and neurological function of the patients with hemiplegia after stroke,and it can help to improve the quality of life.
