肺炎支原体肺炎伴喘息儿童血清25(OH)D_3水平与血清IL-10、VEGF水平及肺功能的相关性 被引量:18

摘要 目的探讨肺炎支原体肺炎伴喘息儿童血清25(OH)D_3水平与血清IL-10(interleukin-10,IL-10)、VEGF(vascular endothelial growth factor)水平及肺功能的相关性。方法选取我院确诊肺炎支原体肺炎(MPP)患儿60例,分为MPP伴喘息组30例,MPP不伴喘息组30例。另选30例正常体检儿童为对照组。酶联免疫方法检测三组血清中IL-10、VEGF的水平。采用化学发光法检测25(OH)D_3水平。并比较肺功能的变化。对25(OH)D_3与IL-10、VEGF、肺功能的相关性进行比较。结果三组比较,25(OH)D_3、IL-10、VEGF的水平差异有统计学意义(P均<0.05)。MPP伴喘息患儿的VEGF的水平高于不伴喘息者,MPP伴喘息患儿的25(OH)D_3、IL-10的水平低于不伴喘息者,差异均有统计学意义(P<0.05)。MPP伴喘息组与MPP不伴喘息组比较,喘息组PEF%、FEV1%、FEV1/FVC明显降低,差异有统计学意义(P<0.05)。Pearson相关性分析结果显示,MPP患儿血清25(OH)D_3水平与IL-10呈正相关,与VEGF水平呈负相关(P均<0.05)。MPP伴喘息组25(OH)D_3水平与PEF%、FEV1%、FEV1/FVC呈正相关(P均<0.05),MPP不伴喘息组25(OH)D_3水平与PEF%、FEV1%、FEV1/FVC未见明显相关性(P>0.05)。结论血清25(OH)D_3水平可能与MPP患儿出现喘息样发作的原因相关,且与其肺功能下降有关。 Objective To evaluate the correlation among serum 25(OH)D3 ,Peripheral IL-10/ VEGF expression and pulmonary?function in children with Mycoplasma pneumoniae pneumonia. Methods Children with MP pneumonia (n=60) were recruited and divided into MPP complicated with wheezing group (n=30) and MPP without wheezing group (n=30). Another 30 healthy children were enrolled as control group. ELISA was used to detect the level of interleukin-10 (IL- 10) and vascular endothelial growth factor ( VEGF). The serum levels of 25(OH)D3 were measured by using chemical luminescence methods. Pulmonary functions were compared between the groups. The correlation relationship among serum 25(OH)D3 and IL-10,VEGF,pulmonary function were thoroughly analyzed. Results The serum levels of 25(OH)D3, IL- 10 and VEGF showed significant differences between groups (p〈0.05). The serum VEGF levels were higher in the MPP complicated with wheezing group than in the MPP withoutwheezing group(P〈0.05),while the serum levels of 25(OH)D3 and IL- 10 in the MPP with wheezing group were lower than in the MPP withoutwheezing group(P〈0.05). Compared with the MPP withoutwheezing group,the PEF% 、FEV1%、 FEV1/FVC in the MPP with wheezing group were decreased(P〈0.05). Pearson correlation analysis showed a positive correlation between 25(OH)D3 and IL-10,while there was a negative correlation between 25(OH)D3 and VEGF in the children with MP pneumonia (P〈0.05). Our study also showed a positive correlation between 25(OH)D3 and PEF% 、FEV1%、 FEV1/FVC in the MPP with wheezing group(p〈0.05). In the MPP withoutwheezing group,there was no correlation between25(OH)D3 and PEF% 、FEV1%、 FEV1/FVC(P〉0.05). Conclusion The serum levels of 25(OH)D3 may play important roles in the pathogenesis of MP pneumonia and wheezing attack in children. It is associated with the decrease of pulmonary?function.
