血清ProGRP和HE4在慢性肾脏病患者中的临床意义 被引量:13

摘要 目的探讨血清胃泌素释放肽前体(pro GRP)和人附睾蛋白4(HE4)的水平变化在慢性肾脏病(CKD)患者中的临床意义。方法选取138例CKD患者和50例体检健康者,用电化学发光法检测血清pro GRP和HE4的水平,用全自动生化分析仪检测血清Scr水平,分析体检健康者和CKD不同分期患者间血清pro GRP、HE4和Scr的水平差异,比较血清pro GRP、HE4与血清Scr的相关性。结果 CKD患者血清pro GRP、HE4和Scr水平随CKD病情的加重而升高,血清pro GRP和Scr水平在CKD2、3、4、5期组间差异均有统计学意义(均P<0.05),血清HE4在CKD各期间差异均有统计学意义(均P<0.05)。血清pro GRP水平与Scr水平呈正相关(r=0.857,P<0.05);血清HE4水平和Scr水平呈正相关(r=0.869,P<0.05)。结论血清pro GRP和HE4水平可以用来评估CKD病情的严重程度,血清HE4在CKD1期即有升高趋势,可作为早期发现和监测肾损害的辅助诊断指标。 ObjectiveTo explore the clinical significance of serum pro-gastrin- releasing-peptide (proGRP) and human epididymis protein 4 (HE4) levels in patients with chronic kidney disease (CKD). Methods The levels of serum proGRP and HE4 were detected by Electrochemiluminescence, while the level of Scr by automatic biochemical method from 138 patients with CKD and 50 healthy subjects. The correlation analysis was performedamong proGRP, HE4 and Scr. ResultsThe levels of serum proGRP,HE4 and Scr in the patients with CKD were significantly higher with the severity of CKD increased:the levels of serum proGRP and Scr in the period of 2,3,4,5 of CKD was statistically significant different(P〈0.05).The levels of serum HE4 in each period of CKD was statistically significant different(P〈0.05).There was positive correlations between serum proGRP concentrations and Scr concentrations(r=0.857, P〈0.05); There was positive correlations between serum HE4 concentrations and Scr concentrations(r=0.869, P〈0.05).Conclusion The levels of serum proGRP and HE4 can be used to assess the severity of CKD, while the serum HE4 increased in 1 period of CKD which can be used as auxiliary diagnostic indices of early detection and monitoring treatment .
