摘要
目的检测妊娠期OGTT试验中不同时间点血糖及胰岛素分泌情况,探讨其水平变化在妊娠期糖尿病诊疗中的临床意义。方法选取2013—2015年于天津市中心妇产科医院就诊的孕期妇女共200例,行口服葡萄糖糖耐量试验(OGTT)及胰岛素释放试验,分为正常(NGT)组80例,妊娠期糖尿病(GDM)组及糖耐量受损(IGT)组各60例。分别检测各组OGTT各时间点(空腹、1 h、2 h)血糖与胰岛素水平。结果与NGT组相比较,GDM组3个时间点血糖水平均升高,差异具有统计学意义(P<0.05);空腹与2 h胰岛素分泌增多,差异有统计学意义(P<0.05),且峰值延迟,2 h左右达到分泌最高峰。IGT组3个时间点血糖较NGT组升高,差异具有统计学意义(P<0.05),2 h胰岛素分泌增多,差异具有统计学意义(P<0.05),分泌峰值未出现延迟。结论妊娠期进行OGTT试验,对于GDM的早期诊断与治疗均具有重要价值,可减少母婴并发症,提高围产医学质量。
Objective To investigate the value and significance of glucose and insulin secretion by OGTT test in diagnosis and treatment of gestational diabetes mellitus (GDM). Methods 200 pregnant women from Tianjin Centre Hospital of Gynecology Obstetrics from 2013 to 2015 were carried out the oral glucose tolerance test (OGTT) and insulin release test. The pregnant women were equally divided into three groups: 80 cases in the group of normal glucose tolerance (NGT), 60 cases in the group of gcstational diabetes mellitus (GDM) and 60 cases in the group of impair glucose tolerance (IGT). The glucose and the insulin were measured at each time point (fasting, 1 h and 2 h) and compared between three groups. Results Compared with the group of NGT, the levels of glucose in three time points were increased in GDM, the difference was statistically significant (P〈0.05). At the time points (fasting and 2 h), the level of insulin were increased, the difference was statistically significant (P〈0.05). The secretion at 2 h was the highest, and the peak value was delayed to release. As to the group of IGT, the levels of glucose in three time points were increased, the difference was statistically significant (P〈0.05). The secretion of insulin at 2 h was increased, the difference was statistically significant (P〈0.05), and the peak value was not delayed to release. Conclusion The detection of OGTT is important for the early diagnosis and treatment of GDM, which could reduce the happening of complications of maternity and infant, and improve the medical quality ofperinatology.
