上前牙外伤冠折后不同粘结方法自体断冠再接的4年随访比较

The clinical efficacy of crown fractured anterior tooth reattached by using different bonding method:a 4-year follow-up

目的:比较上前牙外伤冠折后采用不同粘结方法自体断冠再接的效果。方法:回顾分析2010年1月至2012年12月上前牙外伤冠折后自体断冠再接患者55例(73颗牙),其中树脂粘结组24例(35颗牙)和纤维桩加强组31例(38颗牙)。树脂粘结组采用Super-Bond C&B树脂粘结剂和Rely X^(TM) Unicem Aplicap自粘接树脂型水门汀再接自体断冠,纤维桩粘结组在树脂粘结基础上加用纤维桩加强再接自体断冠。经过4年的随诊观察,采用Kaplan-Meier法绘制生存曲线,采用log-rank检验分析比较2组断冠再接后4年的断冠保留率。结果:纤维桩加强组4年累积断冠保留率高于树脂粘结组,差异有统计学意义(P<0.05)。结论:使用纤维桩加强的断冠再接临床效果好,操作简单快捷,可以减少断冠再接后再次折断的发生。

[Objective] To compare the clinical efficacy of crown fractured anterior tooth reattached by using different bonding method.[Method]73 anterior teeth of 55 patients with crown fractured were selected from Jan 2010 to Dec2012.One Group (35 cases of 24 patients) were rebuilt-up by Super-Bond C&B and Rely X Unicem Aplicap and other Group (38 cases of 31 patients) were strengthened by fiber post.The effect of the different methods were compared in 4-year follow-up,which was analyzed by Kaplan-Meier survival curve and log-rank test.[Result]In 4-year follow-up,survival rate of strengthened filling by fiber post group was significantly higher than that of the group without fiber post (log-rank test,χ~2= 8.340,P = 0.004).[Conclusion] The crown fractured anterior teeth reattached by fiber post are effective,simple and quick.It can greatly increase survival rates.
