三仁汤对脾胃湿热型功能性消化不良患者胃排空和胃肠道激素水平的影响

Effect of Sanren decoction on gastric emptying and gastrointestinal hormone in patients with functional dyspepsia with syndrome of spleen and stomach dampness-heat

目的观察三仁汤对脾胃湿热型功能性消化不良患者胃排空和胃肠道激素水平的影响。方法将62例脾胃湿热型功能性消化不良患者随机分为观察组和对照组。对照组给予枸橼酸莫沙比利片治疗,观察组在此基础上给予自拟三仁汤治疗,2组均以4周为1个疗程,治疗1个疗程后,观察2组治疗前后临床症状评分、胃排空指标及胃肠激素水平变化情况,评估2组疗效。结果治疗后,2组上腹痛、嗳气、饱胀不适、胃纳减少及咽部梗阻感积分均显著降低(P均<0.05),且观察组治疗后单项临床症状积分均显著低于对照组(P均<0.05);治疗后,2组胃半排空时间均显著缩短(P均<0.05),胃窦收缩频率和胃窦收缩幅度均显著升高(P均<0.05),且观察组治疗后各指标改善情况均显著优于对照组(P均<0.05);治疗后,2组胃动素和胃泌素含量均显著升高(P均<0.05),胆囊收缩素含量均显著降低(P均<0.05),且观察组治疗后各指标改善情况均显著优于对照组(P均<0.05);观察组临床总有效率显著高于对照组(P<0.05)。结论三仁汤治疗脾胃湿热型功能性消化不良疗效确切,可有效调节患者胃肠道激素紊乱,改善患者胃肠道功能障碍,且安全性高。

Objective It is to observe the effect of Sanren decoction on gastric emptying and gastrointestinal hormone in patients with functional dyspepsia with syndrome of spleen and stomach dampness-heat. Methods 62 patients with functional dyspepsia with syndrome of spleen and stomach dampness-heat were randomly divided into observation group and control group. The control group was treated with Mosapride Citrate, the observation group was treated with Sanren decoction on the basis of treatment in the control group. Both groups were treated for 4 weeks. The changes of clinical symptom scores, indexes of gastric emptying and gastrointestinal hormones before and after treatment were observed and the curative effects were evalua- ted in both groups. Results After treatment, the scores of abdominal pain, belching, stomach stuffiness, poor appetite and pharyngeal obstruction were significantly lower than those before treatment in both groups （ P 〈 0.05 ）, and the scores in the observation group were significantly lower than those in the control group（ P 〈 0.05 ） ; gastric half emptying times were signifi- cantly shortened （P 〈0. 05）, gastric antrum frequency and distal gastric contraction amplitude were significantly increased in both groups （ P 〈0. 05 ）, and the improvement of every index in the observation group was significantly better than that of control group （P 〈 0.05） ; the contents of motilin and gastrin were significantly increased while that of cholecystokinin was de- creased in both groups （P 〈 0.05） , and the the improvement of every index in the observation group was significantly better than that of control group （P 〈 0.05 ）. The total clinical effective rate in the observation group was significantly higher than that in the control group （ P 〈 0.05 ）. Conclusion Sanren decoction which can effectively regulate the gastrointestinal hormone disorders and improve the gastrointestinal tract dysfunction has definite curative effect on functional dyspepsia with syndrome of spleen and stomach dampness-heat with high safety.
