黄连解毒汤治疗老年痴呆的Meta分析

Meta Analysis of Huanglian Jiedu Decoction in Treatment of Senile Dementia

目的系统评价黄连解毒汤治疗老年痴呆的有效性。方法计算机检索相关文献,按照制定的标准筛选后,由2名研究者单独提取资料、交叉核对,将数据录入Rev Man5.2作统计分析。结果共纳入6个研究,共539例患者,黄连解毒汤联合其他西药或中药为治疗组,单独的西药或中药治疗为对照组。Meta分析结果表明:(1)比较治疗后的总有效率,黄连解毒汤联合其他西药或中药治疗效果明显高于对照组单独中药或西药治疗(OR=3.06,95%CI(1.11,8.47)),差异具有统计学意义(Z=2.16,P=0.03<0.05)。(2)比较两组的认知功能(MMSE评分)(WMD=1.07,95%CI(0.53,1.06)),差异具有统计学意义(Z=3.90,P<0.000 1)。(3)比较两组的日常生活能力ADL评分,治疗组与对照组差异无统计学意义(WMD=-1.01,95%CI(-5.64,3.61),Z=0.43,P=0.67>0.05)。结论黄连解毒汤联合其他西药或中药对老年痴呆患者日常生活能力影响差异无统计学意义,但对认知功能有的改善差异具有统计学意义,此研究表明黄连解毒汤联合其他西药或中药治疗老年痴呆效果显著优于单纯的中药或西药治疗,且无毒副反应,由于其制药方便且价格便宜,值得在临床进一步推广。

Objective To evaluate the effectiveness of Hunaglian Jiedu Decoction in treatment of senile dementia patients systematically. Methods In the first place,the writers retrieved relevant literatureby computer. After screening them according to the established standards,two researchers extracted theinformation independently,and cross checked the information,then input the data to Rev Man5.2 forstatisticanalysis. Results Six researches consisted of 539 patients have been adopted. Treatmentgroup:patients treated by Huanglian Jiedu Decoction in combination with other western medicine ortraditional Chinese medicine. Control group:patients treated by western medicine or traditional Chinesemedicine. The Meta analysis results showed:1)To compare the total effective rate after treatment,thetherapeutic effect of treatment group was obviously higher than that of control group( OR =3. 06,95%CI(1. 11,8. 47)),the difference was statistically significant( Z =2. 16,P =0. 03<0. 05);2)Tocompare the cognitive function of the two groups(MMSE Score)( WMD = 1. 07,95% CI(0. 53,1. 06)),the difference was statistically significant( Z = 3. 90, P < 0. 000 1 < 0. 05);3)To compare the ability of daily life of the two groups(ADL Score),the difference was not statisticallysignificant between the two groups( WMD =1. 01,95% CI(-5. 64,3. 61),Z = 0. 43, P = 0. 67>0. 05). Conclusion The group treated by Huanglian Jiedu Decoction in combination with other westernmedicine or traditional Chinese medicine and the group treated by western medicine or traditionalChinese medicine,the difference is not statistically significant in the impact of daily life ability betweenthem. While in improvement of cognitive function,the difference is statistically significant. Thisresearch indicates that the effects of treatment group are much better than the effects of control group.On account of its pharmaceutical convenience,cheap price and non-toxic and non-side effects,thetherapeutic method is worthy of popularization in clinic.
