肺表面活性物质治疗足月新生儿急性呼吸窘迫综合征疗效分析 被引量:8

摘要 目的探讨肺表面活性物质治疗足月新生儿急性呼吸窘迫综合征的临床疗效。方法选择该院2015-02~2016-04收治的74例足月新生儿急性呼吸窘迫综合征患儿为研究对象,按随机数字表法分为观察组和对照组各37例。对照组接受常规疗法,观察组在对照组基础上加用肺表面活性物质治疗。对两组患儿疗效及治疗前与治疗后24 h血气指标进行观察比较。结果观察组显效15例,有效20例,无效2例。对照组显效9例,有效19例,无效9例。观察组疗效优于对照组(P<0.05)。治疗前,两组患者血气分析指标无明显差异(P>0.05),治疗后观察组血氧分压(Pa O2)、二氧化碳分压(Pa CO2)、p H值均优于对照组,差异有统计学意义(P<0.01)。结论肺表面活性物质应用于足月新生儿急性呼吸窘迫综合征治疗中效果显著,能快速改善血气指标,值得临床推广。 Objective To explore the clinical effects of pulmonary surfactant on treatment of full-term neonatals with severe acute respiratory distress syndrome.Methods 74 full-term newborns with acute respiratory distress syndrome in our hospital from February 2015 to April 2016 were randomly divided into the observation group and the control group by the random number table method, with 37 cases in each group. The control group was given conventional treatment, while the observation group was treated with the same treatment as the control group plus pulmonary surfactant. The clinical effects and 24-hour blood gas indexes were compared between the two groups before and after the treatment.Results In the observation group,15 cases were markedly effective, 20 cases effective and 2 cases ineffective, while in the control group, 9 cases were markedly effective, 19 cases effective, and 9 cases ineffective, with a better clinical effect in the observation group(P〈0.05). Before the treatment, there were no significant differences in the indexes of blood gas analysis between the two groups(P〉0.05). After the treatment, the values of PaO2, PaCO2 and pH in the observation group were improved better than those in the control group(P〈0.01).Conclusion Pulmonary surfactant is effective in the treatment of acute respiratory distress syndrome in full-term neonates.
