摘要
目的 : 探讨较大剂量氯氮平对心电图QT离散度 (QTd)及校正后的QT离散度 (QTcd)的影响 ,从而探讨其可能引起猝死的机制及预防方法。方法 :测定精神疾病患者在氯氮平最高剂量 5 0 0mg/d第 2、4周的QTd及QTcd ,并与健康对照组做对比分析。 结果 :氯氮平组在最高剂量第 2、4周的QTd和QTcd均较对照组明显增大 ,均有非常显著性差异。 结论 :较大剂量氯氮平对心肌复极过程有影响 。
Objective:To observe the effect of higher dosage of clozapine on ECG in QT dispersion (QTd) and post regulating QT dispersion (QTcd), explore the possible mechanism and prevention of sudden death during clozapine treatment. Method:33 cases of patients with mental disorders were treated with clozapine and received ECG examination at 2 and 4 weeks after reaching the maximum dosage of 500mg/d. QTd and QTcd were compared between the patients and 33 healthy adults. Results:QTd and QTcd were significantly increased in patients with higher dosage of clozapine than in the healthy group. Conclusion:Higher dosage of clozapine could influence the cardiomuscular repolarization.ECG examination could be used as a monitor to prevent sudden death in patients treated with higher dosage of clozapine.
