营养风险筛查评估在肺结核伴糖尿病患者中的应用

Application of nutritional risk screening assessment in patients with pulmonary tuberculosis complicating diabetes

目的探讨营养风险筛查评估在肺结核伴糖尿病患者中的应用价值。方法选取2013年6月至2015年6月该院收治的肺结核伴糖尿病患者80例,依据随机数字表法将这些患者分为营养风险筛查评估组(NRS2002组,n=40)和简易营养评价组(MNA组,n=40),对两组患者的营养不良发生情况、NRS2002组中营养不良和无营养不良患者的临床结局进行统计分析。结果两组患者营养不良发生率差异无统计学意义(P>0.05),同时体质量减少、膳食摄入减少、有较为严重的疾病存在、年龄70岁的比例差异无统计学意义(P>0.05);NRS2002组中营养不良患者的并发症发生率明显高于无营养不良患者,住院时间明显长于无营养不良患者,住院费用明显高于无营养不良患者,差异均有统计学意义(P<0.05)。结论 NRS2002和MNA在肺结核伴糖尿病患者中均具有较高的应用价值,NRS2002更简便易行,值得在临床推广。

Objective To investigate the application value of nutritional risk screening assessment in the patients with pulmonary tuberculosis complicating diabetes.Methods Eighty cases of pulmonary TB complicating diabetes in the hospital from June 2013 to June 2015 were selected and divided into the nutritional risk screening assessment group（NRS2002group,n=40）and mini nutritional assessment group（MNA group,n=40）according to the random number table.The incidence situation of malnutrition in the two groups,and clinical outcomes in the patients with malnutrition and without malnutrition of the NRS2002 group were statistically analyzed.Results The incidence rate of malnutrition had no statistically significant difference between the two groups（P〉0.05）,meanwhile proportion had no statistical difference among the patients with body mass decrease,dietary intake reducing,more severe diseases,age over 70 years old（P〉0.05）;the incidence rate of complications in malnourished patients of the NRS2002 group was significantly higher than that in the patients without malnutrition,the hospitalization time was significantly longer than that in the patients without malnutrition,the hospitalization costs were significantly higher than the patients without malnutrition,the differences were statistically significant（P〈0.05）.Conclusion NRS2002 and MNA have higher application values in the patients with pulmonary TB complicating diabetes,NRS2002 is more simple and easy to operate,so is worthy of promotion in clinic.
