电视胸腔镜手术对老年非小细胞肺癌患者术后心肺并发症的影响

Effects of video-assisted thoracoscopic surgery on postoperative cardiopulmonary complications in elderly patients with non-small cell lung cancer

目的:探讨电视胸腔镜手术治疗老年非小细胞肺癌对患者心肺功能及并发症的影响。方法:60例老年非小细胞肺癌患者,采用双盲法分为对照组30例,采用传统常规开胸外科手术治疗;观察组30例,采用电视胸腔镜微创手术治疗。比较两组患者治疗前后左心室收缩末期内径(LVESD)、左心室舒张末期容积(LVEDV)、左心射血分数(LVEF)以及心率等指标以及术后并发症发生情况。结果:手术治疗前,两组患者LVESD、LVEDV指标之间无统计学差异(P>0.05);治疗后,两组患者的两项指标均有不同程度的改善,但组间比较观察组更低(P<0.05);手术前,两组患者的心率、LVEF等心功能指标之间无统计学差异(P>0.05),治疗后,均有一定程度的改善,但观察组更为明显(P<0.05);观察组术后并发症的发生率3.33%,低于对照组的23.33%(P<0.05)。结论:在老年非小细胞肺癌的治疗中,电视胸腔镜手术的安全性明显优于开胸外科手术,在临床实践中可给予优先考虑。

Objective：To study the effect of video-assisted thoracoscopic surgery on postoperative cardiopulmonary function and complications in elderly patients with non-small cell lung cancer（NSCLC）.Methods：60 cases NSCLC were divided into two groups each 30 cases.The control group were treated by traditional conventional thoracic surgery.The observation group were given video micro-invasive surgery for treatment.TheLVESD,LVEDV,LVEF and heart rate were measured,and the postoperative complications were analyzed.Results：After treatment,the levels of LVESD and LVEDV were significantly improved,and the observation group was lower than the control group（P0.05）;After treatment,the heart rate and LVEF of the two groups were significantly improved,and the observation group was better than the control group,the difference was statistically significant（P 0.05）;The probability of postoperative infection complication was significantly lower than that of the control group,the difference was statistically significant（P 0.05）.Conclusion：Video-assisted thoracoscopic surgery in elderly patients with NSCLC can significantly improve heart and lung function,reduce complications,worthy of clinical application and promotion.
