小针刀松解术联合中药熏蒸治疗椎间盘源性下腰痛临床研究

Small needle-knife lysis combined with fumigating with the traditional Chinese medicine in the treatment of discogenic low back pain

目的:探讨小针刀松解术、中药熏蒸联合治疗椎间盘源性下腰痛的临床效果。方法:将86例椎间盘源性下腰痛患者按入院顺序编号随机分为对照组、观察组各43例,两组均采用小针刀松解术治疗,观察组加用中药熏蒸治疗,比较两组治疗效果、视觉模拟评分(VAS)、腰椎功能评分(JOA)及腰痛功能障碍指数评分(ODI)的改善情况。结果:观察组治疗总有效率为93.02%,明显高于对照组的74.42%(P<0.05);治疗1周、治疗1个月,两组VAS评分、ODI评分均降低,与治疗前对比差异有统计学意义(P<0.05),观察组治疗不同时间VAS评分、ODI评分均低于对照组,对比差异有统计学意义(P<0.05);治疗1周、治疗1个月两组JOA评分均上升,与治疗前对比差异有统计学意义(P<0.05),观察组JOA评分上升幅度高于对照组(P<0.05)。结论:采用小针刀松解术联合中药熏蒸治疗椎间盘源性下腰痛患者总有效率高,同时可明显减轻患者疼痛程度,改善其腰椎功能。

Objective：To investigate the clinical effect of small needle-knife lysis combined with fumigating with the traditional Chinese medicine in the treatment of discogenic low back pain.Methods：Eighty-six patients with discogenic low back pain were randomly divided into the control group and the observation group,according to the order of admission,43 cases in each group.Both groups were treated by small needle-knife lysis,and the observation group was additionally treated by fumigating with the traditional Chinese medicine.The curative effect,visual analogue scale（VAS）score,lumbar function score（JOA）and low back pain Oswestry disability inde（ODI）score were compared between the two groups.Results：The total effective rate of the observation group（93.02%）was significantly higher than that of the control group（74.42%）（P0.05）;1 week and 1 month after treatment,VAS scores and ODI scores were lower than those before treatment（P0.05）,and the scores of observation group were lower than those of control group at different time（P0.05）;1 week and 1 month after treatment,JOA scores were higher than those before treatment（P0.05）,and the JOA scores in observation group were significantly higher than those in control group（P0.05）.Conclusion：The total effective rate of small needle-knife lysis combined with fumigating with the traditional Chinese medicine is high in the treatment of discogenic low back pain,and it can significantly relieve pain and improve the lumbar function.
