小儿肺炎雾化吸入治疗中应用品管圈活动的临床效果及对依从性、炎性因子的影响 被引量:23

Clinical effect of quality control circle activities in atomization inhalation treatment of infants with pneumonia and the influence on compliance and inflammatory cytokines

摘要 目的探讨小儿肺炎雾化吸入治疗中应用品管圈活动的临床效果及对依从性、炎性因子的影响。方法 60例肺炎患儿均予以雾化吸入治疗,对照组进行常规干预,研究组基于对照组基础上予以品管圈活动,比较两组治疗后疗效、依从性、临床表现缓解时间、炎症因子及不良反应发生情况。结果治疗后,研究组总有效率为96.67%,高于对照组80.00%(P<0.05)。研究组依从率为93.33%,高于对照组73.33%,差异有统计学意义(P<0.05)。研究组咳嗽、发热、憋喘、肺部啰音缓解时间少于对照组(4.61±0.57)d vs.(5.30±0.66)d、(4.58±0.54)d vs.(5.21±0.62)d、(3.90±0.45)d vs.(4.87±0.60)d、(4.66±0.54)d vs.(5.18±0.63)d,差异均有统计学意义(均P<0.05)。研究组肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)水平低于对照组(41.52±5.11)U/ml vs.(68.33±8.53)U/ml、(140.85±17.50)ng/L vs.(189.31±23.67)ng/L、(0.63±0.07)μg/L vs.(0.89±0.11)μg/L,差异均有统计学意义(均P<0.05)。研究组不良事件发生率为10.00%,低于对照组33.33%,差异有统计学意义(P<0.05)。结论品管圈活动在小儿肺炎雾化吸入治疗中的应用效果肯定,能够提高患儿的依从性,减少不良事件的发生,并可利于炎症反应的改善,促进患儿恢复。 Objective To explore the clinical effect of quality control circle activities in atomization inhalation treatment of infants with pneumonia and the influence on compliance and inflammatory eytokines. Methods A total of 60 infants with pneumonia were treated with atomization inhalation, the infants in control group were treated by regular intervention, and the infants in study group were treated by quality control circle activities on the basis of regular intervention. The efficacy, compliance, clinical manifestation remission time, inflammatory cy- tokines, and adverse reactions after treatment were compared between the two groups. Results After treatment, the total effective rate in study group was 96. 67% , which was statistically significantly higher than that in control group ( 80. 00% ) ( P〈0. 05 ) . The compliance rate in study group was 93.33% , which was statistically significantly higher than that in control group (73.33%) (P〈0. 05) . The remis- sion time of cough, fever, wheeze, and pulmonary rales in study group were statistically significantly less than those in control group [ (4.61±0.57) daysvs. (5.30±0.66) days, (4.58±0.54) daysvs. (5.21±0.62) days, (3.90±0.45) daysvs. (4.87±0.60) days, (4. 66±0. 54) days vs. (5.18±0. 63) days] (all P〈0. 05) . The levels of tumor necrosis factor-α (TNF-α), interleukin-6 ( IL-6), and interleukin-8 (IL-8) in study group were (41.52±5. 11 ) U/ml, ( 140. 85±17.50) ng/L, and (0. 63±0. 07) μg/L, respectively, which were statistically significantly lower than those in control group [ (68. 33±8.53 ) U/ml, ( 189.31 ±23.67) ng/L, and (0. 89±O. 11 ) μg/L, respectively] (all P〈0. 05 ) . The incidence rate of adverse events in study group was 10. 00% , which was statistically sig- nificantly lower than that in control group (33.33%) ( P〈0. 05 ) . Conclusion The application effect of quality control circle activities in atomization inhalation treatment of pneumonia in children is certain, which can improve the compliance of children, reduce the occurrence of adverse events, improve inflammatory response, and promote the recovery of children.
