前列地尔联合羟苯磺酸钙治疗2型糖尿病肾病临床观察

Clinical Observation of Alprostadil Combined with Calcium Dobesilate in Treatment of Patients with Type 2 Diabetic Nephropathy

目的探讨前列地尔联合羟苯磺酸钙治疗2型糖尿病肾病临床效果及外周血胰岛素样生长因子-1(IGF-1)、内皮抑素(ES)和血管内皮生长因子(VEGF)水平的影响。方法选取陕西省安康市中医医院2015年1月—2016年12月收治的2型糖尿病肾病98例,根据治疗方法分为对照组及观察组,每组49例。对照组在常规治疗基础上采用前列地尔治疗,观察组在对照组基础上联合羟苯磺酸钙治疗。比较2组临床疗效、治疗前后外周血IGF-1、ES、VEGF及糖化血红蛋白(Hb A1c)水平及不良反应。结果观察组总有效率高于对照组(P<0.05)。2组治疗后IGF-1、ES、VEGF、Hb A1c水平均低于治疗前,且观察组低于对照组(P<0.05)。2组不良反应总发生率比较差异无统计学意义(P>0.05)。结论前列地尔联合羟苯磺酸钙治疗2型糖尿病肾病临床疗效较好,安全性较高。

Objective To investigate effects of Alprostadil combined with Calcium Dobesilate on clinical effect and levels of insulin-like growth factor-1 （IGF-1）, endostatin （ES） and vascular endothelial growth factor （VEGF） in peripheral blood of patients with type 2 diabetic nephropathy. Methods A total of 98 patients with type 2 diabetic ne-phropathy admitted during January 2015 and December 2016 were divided into control group and observation group （ n=49 for each group） according to treatment methods. Control group was treated with Alprostadil on the basis of conventional therapy , while observation group was treated with Calcium Dobesilate on the basis of the treatment for control group. Clin-ical efficacy, IGF-1, ES, glycosylated hemoglobin （HbA1c） and VEGF levels in peripheral blood before and after treat-ment and incidence rate of adverse reactions were compared between the 2 groups. Results The total effective rate in observation group was significantly higher than that in control group （P〈0. 05）. After treatment, IGF-1, ES and VEGF and HbAlc levels were significantly lower than those before treatment in two groups, and the levels in observation group were significantly lower than those in control group （P〈0. 05）. There was no significant difference in the total incidence rate of adverse reactions between the two groups （P〉0. 05）. Conclusion Alprostadil combined with Calcium Dobesil-ate can achieve good effect and safe in treatment of patients with type 2 diabetic nephropathy.
