摘要
目的:应用声脉冲辐射力成像技术(ARFI)测量慢性肾病组和健康对照组的双肾肾皮质的剪切波速度(SWV),探讨ARFI技术对慢性肾病定量分期的诊断价值。方法:将健康体检者作为对照组,慢性肾病患者依据K/DOQI分级标准分为1~5期组,对80例慢性肾病患者及20例健康体检者肾脏分别进行ARFI测量获得SWV值并进行统计学分析比较。结果:对照组和不同慢性肾病分期组受检者的SWV值差异有统计学意义(F=137.152,P<0.001)。不同分期组慢性肾病患者的SWV值均低于健康对照组,差异有统计学意义(P<0.001)。方差分析比较不同慢性肾病分期组患者的SWV值,F=82.038,P<0.001,表示不同分期组SWV整体间有差异。进一步多重比较,结果显示任意两个分期组间SWV值比较,差异均有统计学意义(P<0.001)。结论:ARFI技术可以定性定量诊断慢性肾病;SWV值在慢性肾病的分期诊断方面敏感性高,可以作为慢性肾病分期的诊断依据。
bObjective: Through the measurement of the shearing wave speed with chronic kidney disease in the different stages by acoustic radiation force impulse(ARFI),to analyze the feasibility of diagnosing chronic kidney disease in the different stages.Methods: The health persons were treated as the control group,patients with chronic kidney disease on the basis of K/DOQI classification standard were divided into phase 1 to 5 groups.The SWV of 80 patients with chronic kidney disease and 20 health checkers were measured,the results were then analyzed statistically.Results: Comparison results showed that SWV of healthy controls and different stages chronic kidney disease group were different,the difference was statistically significant(F = 137.152,P〈0.001).The SWV of different stages of chronic kidney disease patients were lower than that of healthy control group,the difference was statistically significant(P〈0.001).Compared the SWV of different stages of chronic kidney disease patients,the analysis results showed that F = 82.038,P〈0.001,there were differences between different stages patients.Further multiple comparison showed that the SWV between any two groups had statistical significance(P〈0.001).Conclusion: ARFI technology is not only qualitative but also quantitatively in diagnosing chronic kidney disease.The sensitivity of SWV in chronic kidney disease staging diagnosis is high,it can be used as a diagnostic basis for chronic kidney disease.
