Pharmaceutical care for the patient with diabetic foot infection complicated with hepatic and renal dysfunctions

目的:探讨临床药师对于糖尿病足感染合并肝、肾功能不全患者的规范化服务方式和模式。方法:临床药师通过参与一例糖尿病足感染合并肝、肾功能不全患者的治疗过程,对患者的降糖药物、抗菌药物、抗乙肝病毒药物等方面提出参考意见,并提供个体化药学服务。结果:临床药师通过全程药学服务,及时发现并解决相关药物治疗问题,促进临床合理用药。结论:临床药师融入治疗团队,开展药学服务,有利于促进临床合理用药。

Objective：To investigate methods and modes of standard pharmaceutical care for the patients with type 2 diabetes complicated with hepatic and renal dysfunctions.Methods：Through active involvement in the treatment process of 1 case of type 2 diabetes complicated with hepatic and renal dysfunctions,clinical pharmacists offered good advice on the administration of anti-diabetic drugs,anti-bacterial and anti-hepatitis B virus drugs,and above all provided individualized pharmaceutical care.Results：Through integrated pharmaceutical care,clinical pharmacists duly found out and solved related problems concerning drug treatment,thus promoting rational medication clinically.Conclusion：Integration of clinical pharmacists into the medical care team and active pharmaceutical care could contribute to the promotion of rational medication clinically.
