临床药师参与一例肝移植术后病人的多学科会诊

Clinical pharmacist involvement in multidisciplinary consultation for a patient after liver transplantation

目的:探讨临床药师参与重症病人多学科会诊的实践意义。方法:临床药师参与1例肝移植术后病人的多学科会诊,针对腹泻和感染的药物治疗,以及免疫抑制剂的剂量调整提出了药学建议,为临床治疗提供帮助。结果:临床医师采纳了药师的药物治疗方案,病人病情趋于稳定。结论:临床药师在复杂病例会诊中,针对药物治疗提供药学服务,有利于保障病人安全、合理、个体化给药。

Objective：To investigate the significance of clinical pharmacist involvement in the multidisciplinary consultation for critically ill patients.Methods：Clinical pharmacists participated in the multidisciplinary consultation for a critical patient after liver transplantation,provided advice on the drug treatment of diarrhea and infection,and adjustment of treatment dosages of immunosuppressants,which was of much help to clinical treatment.Results：Clinical physicians accepted the pharmacist drug treatment scheme and the patient conditions became stable.Conclusion：In clinical consultation of complicated cases,pharmacists could provide pharmaceutical care in accordance with the patient conditions and drug treatment,and could ensure the safety,rationality and individualized medication of patients.
