吡喹酮辅助常规护肝干预治疗晚期血吸虫病肝纤维化50例

Clinical Study of Efficacies of Praziquantel in the Auxiliary Therapeutic Intervention of Hepatic Fibrosis Caused by Advanced Schistosomiasis

目的:探讨吡喹酮辅助常规护肝干预治疗晚期血吸虫病肝纤维化临床疗效。方法:选取医院2015年5月至2017年5月收治晚期血吸虫病肝纤维化患者100例,以随机数字表学分为对照组(50例)和观察组(50例),分别给予常规护肝干预和在此基础上加用吡喹酮辅助治疗;比较两组患者治疗前后肝纤维化分级、门静脉内径、脾静脉内径、脾厚度及血清学指标水平。结果:观察组患者治疗后肝纤维化水平均显著低于对照组、治疗前(P<0.05);观察组患者治疗后门脾静脉内径和脾实质厚度均显著优于对照组、治疗前(p<0.05);观察组患者治疗后ALT、AST、HA及LN水平均显著低于对照组、治疗前(P<0.05)。结论:吡喹酮辅助常规护肝干预治疗晚期血吸虫病肝纤维化可有效延缓肝硬化进程,保护肝脏功能,并有助于改善血清学指标。

Objective:To investigate the clinical efficacy of praziquantel in the treatment of advanced schistosomiasis hepatic fibrosis.Methods:A total of 100 patients with schistosomiasis hepatic fibrosis,who were treated in our hospital during May 2015 to May 2017,were selected and divide into an observation group and control group randomly,with 50 cases each group.The patients in the control group were given conventional hepatic protective medication,whereas those in the observation group were given additional praziquantel on the basis of conventional therapy.Hepatic fibrosis grading,portal vein diameter,splenic vein diameter,spleen thickness and serum level were compared between the two groups before and after treatment.Results:The level of hepatic fibrosis in the observation group after the treatment was significantly lower than that in the control group(P<0.05),the diameter of the splenic vein and the thickness of the spleen parenchyma in the observation group were significantly better than those in the control group(P<0.05),and the levels of ALT,AST,HA and LN in the observation group were significantly lower than those in the control group(P<0.05).There were also significant differences in the level of hepatic fibrosis,the diameter of the splenic vein,the thickness of the spleen parenchyma,and the levels of ALT,AST,HA and LN in the control group before and after treatment(P<0.05).Conclusion:Conventional hepatic protective medication combined with Praziquantel can effectively retard the progression of cirrhosis,protect liver function and improve serological indexes in the treatment of hepatic fibrosis due to advanced schistosomiasis.
