牙周治疗对慢性牙周炎伴冠心病患者血清CRP、IL-8和TNF-α水平的影响

Effects of periodontal therapy on the levels of serum CRP,IL-8 and TNF-α in coronary heart disease patients with chronic periodontitis

目的探讨牙周治疗对慢性牙周炎伴冠心病患者血清中C-反应蛋白(CRP)、白介素-8(IL-8)及肿瘤坏死因子-α(TNF-α)水平的影响及其干预效果。方法选取46例伴有冠心病的慢性牙周炎患者,随机分为观察组和对照组,各23例。观察组接受常规心内科治疗和牙周治疗,对照组接受常规心内科治疗。观察比较两组治疗前后的牙周临床指标与血清CRP、IL-8和TNF-α水平。结果观察组经牙周治疗后PLI、BI、PD、CAL均较治疗前下降,差异有统计学意义(P均<0.05)。治疗前,两组患者血清中CRP、IL-8及TNF-α水平比较,差异均无统计学意义(P>0.05);治疗后,观察组CRP、IL-8和TNF-α水平均降低,且观察组低于对照组,差异有统计学意义(P<0.05)。结论牙周治疗能改善慢性牙周炎伴冠心病患者的牙周状况,降低其血清CRP、IL-8和TNF-α水平,有利于冠心病的预防和治疗。

Objective To investigate the effect of periodontal therapy on serum C-reactive protein (CRP),interleukin-8 (IL-8) and tumor necrosis factor-α (TNF-α) in patients with chronic periodontitis and coronary heart disease.Methods Forty-six patients with chronic periodontitis with coronary heart disease were randomly divided into observation group and control group.The observation group received routine cardiac surgery and periodontal treatment,the control group received conventional cardiac surgery.The levels of serum CRP,IL-8 and TNF-α were observed and compared between the two groups before and after treatment.Results After periodontal therapy,all the periodontal clinical indices of experimental group became better and the serum levels of CRP,IL-8 and TNF-α were lower than control group,and the difference was significant (P <0.05).Conclusion Periodontal therapy can improve the periodontal status of patients with chronic periodontitis and coronary heart disease and reduce the levels of serum CRP,IL-8 and TNF-α,which is beneficial to the prevention and treatment of coronary heart disease.
