CEA、CA153和CRP对乳腺癌的诊断价值研究

Diagnostic value of CEA,CA153 and CRP in breast cancer

目的探讨癌胚抗原(CEA)、糖链抗原153(CA153)和C反应蛋白(CRP)联合检测对乳腺癌的诊断价值。方法选取2014年1月至2016年1月濮阳市中医院收治的98例乳腺癌患者作为癌症组,选择同期98例良性乳腺疾病患者作为良性组,98例健康人作为正常组,检测3组患者的血清CEA、CA153和CRP水平,统计分析其诊断价值。结果癌症组的血清CEA、CA153和CRP水平均高于良性组、正常组,差异有统计学意义(P均<0.05);良性组的血清CA153和CRP水平均高于正常组,差异有统计学意义(P均<0.05);良性组的血清CEA水平与正常组比较,差异无统计学意义(P>0.05)。CEA、CA153及CRP的ROC曲线下面积分别为0.718、0.664、0.783;二元Logistic回归分析CEA、CA153和CRP联合检测的灵敏度与特异度为0.908、0.927,ROC曲线下面积为0.946,高于各项指标单独检测的灵敏度及特异度,差异有统计学意义(P均<0.05)。结论联合检测血清CEA、CA153和CRP水平,可有效提高乳腺癌诊断的灵敏度和特异度,临床价值较高,值得推广应用。

Objective To investigate the diagnostic value of combined detection of carcinoembryonic antigen,carbohydrate antigen 153 and C reactive protein in breast cancer.Methods Ninety-eight patients with breast cancer from January of 2014 to January of 2016 in Puyang City Hospital were selected as the cancer group.Ninety-eight patients with benign breast disease were selected as benign group and ninety-eight healthy subjects as the normal group.Three groups of patients Serum CEA,CA153 and CRP levels,statistical analysis of its diagnostic value.Results The levels of serum CEA,CA153 and CRP in the cancer group were significantly higher than those in the benign group and the normal group (P < 0.05).The levels of serum CA153 and CRP in the benign group were higher than those in the normal group,the difference was statistically (P < 0.05).There was no significant difference in serum CEA level between benign group and normal group (P > 0.05).The area under the ROC curve of CEA,CA153 and CRP was 0.718,0.664 and 0.783 respectively.The sensitivity and specificity of CEA,CA153 and CRP were 0.908 and 0.927,and the area under the ROC curve was 0.946 (P < 0.05).The sensitivity and specificity of the individual indexes were statistically significant (P < 0.05).Conclusion The combined detection of serum CEA,CA153 and CRP levels can effectively improve the sensitivity and specificity of breast cancer diagnosis,the clinical value is high,worthy of popularization and application.
