PVP治疗OVCF高粘度与普通骨水泥的疗效对比研究

摘要 目的:对比研究经皮椎体成形术(PVP)在治疗椎体压缩性骨折(OVCF)中应用高粘度骨水泥与普通骨水泥的临床效果及并发症.方法:选取2014年8月-2016年4月入院行经皮椎体成形术手术治疗的80例骨质疏松性椎体压缩性骨折患者按照治疗方法将其分为高粘度骨水泥组与普通骨水泥组,各40例.比较2组术前术后VAS疼痛评分,SF-36生活质量评分,椎体高度恢复情况,手术并发症评估.结果:2组患者术后VAS评分、SF-36生活质量评分明显优于术前,差异有统计学意义(P〉0.05);2组患者术后VAS评分、SF-36生活质量评分比较无明显差异;高粘度骨水泥组患者治疗后Cobb角恢复显著高于治疗前,差异有统计学意义(P〉0.05);高粘度骨水泥组患者骨水泥渗漏发生率显著低于普通骨水泥组患者(8.00%vs 40.00%),差异有统计学意义(P〈0.05).结论:高粘度骨水泥可显著改善Cobb角,减少VAS评分,提高SF-36生活质量,并降低骨水泥渗漏发生率. Objective: To research the comparafion clinical effects and complications between the high viscosity bone cement and ordi- nary bone cement in pereutaneons vcrtebreplasty for treatment of osteoporotic Vertebral compression fractures. Methods: From August 2014 to April 2016,80 patients with OVCF were cnro]]ed in this study. According to the viscosity of the bone cement and surgical ap- proach divided the patients into two groups: PVP by high viscosity bone cement(40 patients) and PVP by ordinary bone cement(40 pa- tients). Compare the clinical effects by the back pain visual analogue scale(VAS) and SF -36 quality of life scores( SF -36 )after surger- y, recovery rate of the average height of the vertebral, the postoperative complications. Results: The postoperative VAS scores and SF- 36 quality of life scores were better than preoperative, with significant differences( P 〈0.05 ) ; there are no significant difference in VAS scores and SF - 36 quality of life scores between the patients in high viscosity bone cement group and the ordinary cement group after treat- ment( p 〉0.05 ). Cobb angles of the patients in high viscosity bone cement group were significantly lower than those in ordinary cement group and those before treatment, and the differences were significant( p 〉 0.05 ). The incidences of cement leakage of patients in high vis- cosity bone cement group were significantly lower than those of patients in ordinary cement group, and the differences were significant ( p 〉 0.05 ). Conchlsion: The high viscosity bone cement could significantly improve the Cobb angle, improve SF - 36 quality of llfe score and reduce VAS score and the incidence of cement leakage.
