结核病患者外周血T细胞亚群检测的临床价值研究

Detection of T cell subsets in peripheral blood of patients with tuberculosis and the research on its clinical value

目的探讨结核病患者外周血T细胞亚群检测的临床价值。方法 2014年10月-2016年10月入住本院的50例结核病患者作为观察组,另外同期选择55例呼吸系统非结核杆菌感染患者作为对照组,选取同期50例健康体检者作为健康对照组,比较3组外周血T淋巴细胞亚群绝对含量及百分含量。结果健康对照组外周血T淋巴细胞亚群绝对含量均高于观察组与对照组,差异有统计学意义(P<0.05)。健康对照组DNT、DPT百分含量均高于观察组与对照组,对照组CD3^+CD4^-CD8^+百分含量显著低于观察组,差异有统计学意义(P<0.05),但3组在CD3^+、CD3^+CD4^+CD8^-百分含量方面差异均无统计学意义(P>0.05)。结论结核病患者外周血T细胞亚群特别是DNT及DPT亚群的检测,对准确判定患者免疫功能状况、病情以及治疗方案的选择等方面具有重要的临床价值。

Objective To investigate the clinical value of T cell subsets detection in peripheral blood of patients with tuberculosis. Methods 50 patients with tuberculosis who were admitted to our hospital from October 2014 to October 2016 were selected as the observation group. 55 patients with non-Mycobacterium tuberculosis infection were selected as the control group. 50 patients were randomly selected as the healthy control group. The absolute contents and percentages of T lymphocyte subsets in peripheral blood were compared among the 3 groups. Results The absolute contents of T lymphocyte subsets in peripheral blood in the healthy control group were significantly higher than those in the control group,with the differences statistically significant（ P〈0. 05）. The percentages of DNT and DPT in the healthy control group were significantly higher than those in the tuberculosis group and the control group,while the percentages of CD3^＋CD4^-CD8^＋in the healthy control group were significantly lower than that in tuberculosis group,with the differences statistically signifiant（ P〈0. 05）,but there was no statistical significance on the difference in CD3^＋,CD3^＋CD4^＋CD8^-among the 3 groups（ P〈0. 05）. Conclusion The detection of peripheral blood T cell subsets,especially the detection of DNT and DPT subsets,in patients with tuberculosis is of great clinical value in accurately determining the immune function,the condition and the choice of treatment regimen.
