摘要
目的:探讨分析能谱CT鉴别诊断腮腺良恶性肿瘤的应用价值。方法:将50例腮腺肿瘤患者纳入本次研究工作,均进行能谱CT检查,并对检查结果进行分析探讨。结果:共发现有58个病灶(46个良性病灶及12个恶性病灶);腮腺良恶性肿瘤的CT征象有明显差异(P<0.05);平扫期及静脉期腮腺良恶性肿瘤的碘浓度值、能谱曲线斜率及有效原子序数均无明显差异(P>0.05);动脉期腮腺良恶性肿瘤的碘浓度值、能谱曲线斜率及有效原子序数有明显差异(P<0.05)。依据动脉期碘浓度阀值(>18.90*100ug/m)进行诊断腮腺恶性肿瘤,结果显示敏感度为81.7%、特异性为79.5%。结论:应用能谱CT鉴别诊断腮腺良恶性肿瘤的临床价值显著,可以起到有效鉴别作用,值得临床加强推广及应用。
Objective: to explore the clinical value of differential diagnosis of parotid benign and malignant tumors by spectroscopy CT.Methods: 50 cases of parotid tumor patients were included in this study,all of them were examined by energy spectrum CT,and the results were analyzed and discussed.The results were found in 58 lesions (46 benign lesions and 12 malignant lesions); CT signs of benign and malignant parotid tumors were significantly different (P 0.05); iodine concentration in unenhanced phase and venous phase of benign and malignant parotid tumors,energy spectrum curve slope and the effective atomic number had no significant difference (P 0.05); iodine concentration in arterial phase of benign and malignant parotid tumors,energy spectrum curve slope and the effective atomic number had significant difference (P 0.05).According to the arterial iodine concentration threshold ( 18.90* 100ug/m),the malignant tumor of parotid gland was diagnosed,the sensitivity was 81.7% and the specificity was 79.5%.Conclusion: the clinical value of differential diagnosis of parotid benign and malignant tumors with energy spectrum CT can be effectively identified,and it is worthy of clinical promotion and application.
