摘要
目的通过超声造影手段观察乳腺浸润性导管癌(IDC)血流动力学参数与其预后分子标记物雌激素受体(ER)、人类表皮生长因子受体2(HER2)间的相关性联系。方法回顾性分析手术病理确诊的60例IDC患者的92个肿块,根据ER和HER2免疫组织化学结果分为"-"、"+"、"++"及"+++"组,对比其与超声造影血流动力学各参数之间的相关性。采用stata11.0软件进行统计分析,不同ER及HER2表达组间超声造影定量参数比较以均数±标准差(x珋±s)表示,对计量资料经正态性检验及方差齐性检验后采用方差分析;利用等级相关系数分析超声造影参数值与ER及HER2级别间是否存在相关性,所有统计学分析均以P<0.05为有统计学意义。结果预后标记物ER中各组对比,超声造影参数中AI、DV、AT及TTP数值有统计学意义的差异,其中AI与DV数值变化与组别的递增呈明显正相关,AT与TTP数值变化与组别的递增呈明显负相关;预后标记物HER2中的各组对比,超声造影参数中RS及CT数值差异有统计学意义,其中RS数值变化与组别的递增呈明显负相关,CT数值变化与组别的递增呈明显正相关。结论超声造影血流动力学定量分析与ER及HER2有较好的相关性,可作为临床IDC诊断的有效补充,尤其能够有效无创观察IDC局部血流灌注情况并对患者的治疗式及预后提供有价值的信息。
Objective To observe the correlation betweencontrast -enhanced ultrasound hemodynamic parameters and expression of ER&amp;HER2 in breast infiltrative ductal carcinoma . Methods Analysed retrospectively the 92 mass in 60 patients with IDC which diagnosed by operation pathology , according to ER and HER2 immunohistochemistry were divided into " -" and " +" , " ++" and " +++" group, compared with the correlation between the hemodynamic parameters of contrast -enhanced ultrasound.Stata 11.0 software was used for statistical analysis , different ER and HER2 expression between quantitative parameters of contrast-enhanced ultrasound compared to standard deviation ( -x ±s) said, the measurement data after test of normality and homogeneity of variance using variance analysis ;Rank correlation coefficient was used to analyze the correlation between the value of contrast -enhanced ultrasonography and the level of ER and HER2, all statistical analysis was statistically significant with P 〈0.05. Results Comparison of the prognostic markers ER in each group, the AI、 DV、 AT and TTP numerical difference was statistically significant, AI and DV numerical change and the category of increasing was significantly positive correlation , AT and TTP numerical change increasing was significantly negative correlation ;Comparison of each group in prognostic marker HER2, the RS and CT numerical difference was statistically significant , RS numerical change and the category of increasing was significantly negative correlation , CT numerical change increasing was significantly positive correlation . Conclusion Contrast-enhanced ultrasound hemodynamic quantitative analysis with ER and HER2 have good correlation, it can be the complement of clinical IDC diagnosis, it can observe the local blood perfusion noninvasively and effectively , and provide valuable information for treatment and prognosis .
