摘要
目的分析临床样本中解脲脲原体(UU)对大环内脂类抗生素的耐药特征,明确UU耐药机制研究方向;方法收集943例疑似UU感染样本,液体培养法鉴定UU感染及耐药情况,分析交沙霉素、螺旋霉素、罗红霉素、阿奇霉素、克拉霉素的耐药特征及交叉耐药关系;结果 943例样本中,检测到耐大环内酯类抗生素的菌株230例,其中螺旋霉素耐药率最高,交沙霉素最低;各抗生素耐药程度构成比存在差异(P<0.01),但与感染人群年龄无关(P>0.05);5种大环内脂类抗生素间存在交叉耐药,大环内脂类抗生素与喹诺酮类抗生素间也存在一定的交叉耐药,且耐药率高的抗生素在其他抗生素的耐药株中耐药明显。结论 UU对不同大环内酯类抗生素耐药率、耐药程度分化明显,显示出复杂、多样的耐药机制,缺少主要的共同耐药机制。
Objective Analysis of drug resistance of Ureaplasma urealyticum to macrolide antibiotics in clinical samples, to clear the research direction of UU - resistant mechanism. Methods 943 suspected cases of UU infection were collected, the UU infection and drug resistance were identified by liquid cultivation, and the drug resistance and cross resistance of Josamycin, Spiramycin, Roxithromycin, Azithromycin and Clarithromycin were analyzed. Results Among the 943 samples, 230 strains were resistant to macrolide antibiotics. The resistant rate of spiramycin wasthe highest, and josamycin was the lowest. The constituent ratio of the resistance degrees were significant differences among the five macrolide antibiotics, but shew no relationship with ages. The cross - resistance among the five macrolide antibiotics was exsit, and there also has cross - resistance between macrolide antibiotics and quinolone antibiotics. The antibiotics with higher resistance rate were more serious cross - resistantto other antibiotics too. Conclusion The drug resistance rate and drug resistance degree of UU were significant differences for different macrolide antibiotics, showing a complex and diverse mechanism of drug resistance, lack of main resistance mechanism.
