参麦注射液改善肺切术后呼吸功能不全临床疗效

The Clinical efficacy of Shenmai injection in the adjunctive treatment of patients with respiratory insufficiency

目的评价参麦注射液辅助治疗肺切除术后呼吸功能不全(respiratory insufficiency,RI)的临床治疗效果,同时探讨参麦注射液对患者白介素-8(interleukin-8,IL-8)以及血清超敏C反应蛋白(high-sensitivity C-reactive protein,hsCRP)水平的影响程度。方法将62例进行肺切除术后的CRI患者,随机分为对照组和治疗组,每组各31例,两组患者均采取给予改善呼衰的常规药物方式进行治疗,治疗组在标准药物治疗基础上静滴参麦注射液,两组均治疗14d,检测、记录和比较两组患者治疗前和治疗后的第1秒用力呼气末容积占用力肺活量百分比(FEV1/FVC)以及血清IL-8、hsCRP水平。结果对照组和治疗组患者的肺功能临床治疗总有效率计算结果分别为94%和100%,治疗组数据显著高于对照组(P<0.05)。治疗后,两组患者的FEV1、FEV1/FVC、PaO_2、PaCO_2检测结果均较治疗前明显好转(P<0.05),且治疗组肺功能及血气指标改善程度与对照组比较有统计学差异(P<0.05)。治疗后,两组患者的血浆IL-8以及hsCRP水平与治疗前比较均较显著下降(P<0.01),且组间数据比较显示,治疗组炎症指标明显好于对照组(P<0.05)。在治疗过程中两组患者均未出现严重不良事件。结论参麦注射液在常规药物治疗基础上辅助治疗慢性呼吸功能不全,可进一步降低患者血清hsCRP、IL-8水平,改善呼吸功能。

Objective To evaluate the clinical efficacy of shenmai injection and investigate its effect on the serum levels of interleukin -8 （IL- 8 ） and. high- sensitivity C -reactive protein（hsCRP） in the adjunctive treatment of patients with chronic congestive respiratory insuffiency. Methods 62 patients with RI enlisted in the study. All the patients were randomly divided into the control group （31 cases） and the treatment group with Shenmai injecta （31 cases） . The FEV1/FVC were detected and the serum levels of IL - 8and hsCRPwere evaluated, before and 2 weeks after treatment. Results After treatment, the total effective rates were respectively 94% and 100% of the control group and treatment group, which was significantly higher in the treatment group（ P 〈 0.05 ）, the FEV1, FEV1/FVC,PaO2,PaCO2 were increased significantly after treatment in two groups （P 〈 0.05） , and between two groups there were significant differences（ P 〈0.05 ）. The levels of IL- 8 and hsCRP decreased in both two groups after treatment（ P 〈 0. 01 ） , although the treatment group was more significant than control group（ P 〈 0. 05 ）. Conclusion It was demonstrated that the elincial efficacy of Shenmai injection was superior to the conventional treatment in the adjunctive treatment of patients with chronic congestive respiratory insuffiency, and Shenmai injection could significantly decrease the serum levels of hsCRP and IL - 8.
