摘要
目的探讨分析IL-23和IL-17在重症肺炎患者外周血的表达及连续血液滤过对其的影响;方法选取70例重症肺炎患者作为研究对象,同时选取70名健康人群作为对照组,比较其外周血IL-23和IL-17水平,同时分析病例组外周血IL-23与IL-17水平相关性,将70例重症肺炎患者随机分为A、B两组,A组患者给予连续血液滤过治疗,B组患者给予常规治疗,比较A、B两组患者治疗前后外周血IL-23和IL-17水平;结果病例组患者外周血IL-17和IL-23水平分别为(95.97±6.43)pg/ml、(74.68±4.73)pg/ml较正常人群的(51.49±6.65)pg/ml、(40.45±5.74)pg/ml,差异均具有统计学意义(P<0.05),病例组患者IL-17和IL-23水平分别为(95.97±6.43)pg/ml、(74.68±4.73)pg/ml,经Pearson直线相关分析显示:病例组外周血IL-17和IL-23水平呈现正相关(r=0.851,P<0.001),治疗前A、B两组患者外周血IL-17和IL-23水平差异无统计学意义(P>0.05),治疗后A、B两组患者外周血IL-17和IL-23水平均显著下降,且A组患者外周血IL-17和IL-23水平下降更为显著,差异具有统计学意义(P<0.05);结论 IL-23及IL-17参与重症肺炎发生和发展,发挥一定的免疫作用,同时连续血液滤过可显著降低重症肺炎患者的外周血中IL-23及IL-17水平,为重症肺炎的监测及治疗的新靶点提供理论依据,提高重症肺炎的治愈率,降低重症肺炎的致死率,在减轻患者疾苦及降低社会及患者医疗负担均有重要的意义。
Objective To investigate the expression of IL - 23 and IL - 17 in peripheral blood of patients with severe pneumonia and the effect of continuous hemofiltration. Methods To select 70 cases of severe pneumonia as the research object, and to select 70 healthy people as the control. To compare the levels of IL -23 and IL - 17 in peripheral blood. To analysis the correlation between IL - 23 and IL - 17 levels in peripheral blood. To divide 70 patients with severe pneumonia into two groups randomly : group A and group B received continuous hemofihration therapy. Group B patients were treated with routine therapy. Compared with group A and group B, IL-23 andIL-17 Level. Results The levels of IL-17 and IL-23 in peripheral blood were (95.97 ± 6.43) pg/mland (74.68 ±4.73) pg / ml, respectively, compared with the normal population (51.49± 6.65) pg / ml, 5.74) pg / ml, the difference was statistically significant ( P 〈 0.05 ). The levels of IL - 17 and IL - 23 in the patients were (95.97 ± 6.43 ) pg / ml and (74.68 ± 4.73 ) pg / ml respectively. The Pearson linear correlation analysis showed that IL - 17 and IL - 23 ( r = 0. 851, P 〈 0. 001 ). There was no significant difference in IL - 17 and IL -23 levels between the two groups before and after treatment (P 〉 0.05). After treatment, the levels of IL - 17 and IL - 23 in peripheral blood of patients with A and B were significantly decreased, and the levels of IL - 17 and IL - 23 in peripheral blood of group A were significantly lower than those of group A ( P 〈 0.05 ). Conclusion IL - 23 and IL -17 may play an important role in the development and progression of severe pneumonia, and continuous hemofihration can signifi- cantly reduce the levels of IL - 23 and IL - 17 in peripheral blood of patients with severe pneumonia Monitoring and treatment of new targets to provide theoretical basis to improve the cure rate of severe pneumonia and reduce the mortality rate of severe pneumonia in reducing patient suffering and reduce the social and medical burden of patients are of great significance.
