摘要
目的研究超声引导下小儿肠套叠温水灌肠复位术前、术中及术后的声像图特征,提高其应用价值。方法选取我院2014年9月至2016年10月在超声引导下肠套叠温水灌肠复位术患儿78例,回顾分析整个灌肠复位过程中超声声像图的演变,总结其各阶段声像图特征。结果 78例肠套叠患儿中成功复位68例,成功率为87.18%,10例复位失败者均为肠套叠合并有阑尾炎。78例患儿术前超声都诊断为肠套叠,确诊率100%,声像图上成功复位68例患儿表现出术前、术中及术后有不同的声像图特征,10例复位失败患儿表现出术前及术中有不同的声像图特征。结论超声引导下小儿温水灌肠复位术各阶段声像图呈不同表现,提高其认识,可以正确评估患儿病情进展,降低患儿治疗风险,减轻患儿痛苦,缩短患儿住院时间,给临床医生诊治提供重要依据。
Objective To study ultrasonographic features of ultrasound guided intussusception reset with warm water enema in the preoperative, intraoperative and postoperative, the aim is to improve its clinical value. Methods 78 eases of children undergoing ultrasound - guided intussusception enema were enrolled in our hospital from September 2014 to October 2016, and the ultrasonographie changes of the whole enema were retrospectively analyzed and the sonographic features of each stage were summarized. Results 68 eases were treated successfully in 78 eases of pediatric intussusception, so the success rate was 87.18% , and the remaining 10 eases reset losers are intestinal composite with appendicitis. 78 cases of preoperative ultrasound are diagnosed as intussusception, which its diagnosis rate is 100%. Ultrasound images of 68 cases of successful ireset have different sonographic features in the preoperative, intra- operative and postoperative, 10 eases children with reset failure showed preoperative and intraoperative different sonographic features. Conclusion Under the guidance of ultrasound warm water enema reduction of each stage sonogram showed different ultrasonographic performance, it can correctly evaluate the progress of children with the disease, reduce the treatment risk, reduce the pain of patients, shorten the time of hospitalization, provide an important basis for clinical diagnosis and treatment.
