通癃启闭汤干预对良性前列腺增生患者生存质量影响的研究

Effect of Tonglong Qibi decoction on quality of life of patients with benign prostatic hyperplasia

目的:观察通癃启闭汤治疗对良性前列腺增生(BPH)患者生活质量的影响及不良反应。方法:将50例BPH患者随机分为治疗组与对照组,治疗组(25例)口服通癃启闭汤,对照组(25例)口服非那雄胺片。观察治疗前后患者的国际前列腺症状评分(IPSS)、国际勃起功能评分(IIEF-5)、排尿症状对生活质量评分(QOL)、膀胱残余尿量(PRV)、前列腺体积(PV)及两组并发症发生率、副作用等。结果:治疗组总有效率为92.00%(95%CI=73.95%~99.05%),对照组为72.00%(95%CI=54.40%~89.60%),两组综合疗效比较(u=1.685 2,P=0.092 0),差异接近临界状态;与对照组比较,治疗组IPSS、QOL及PRV参数降低(P<0.05),IIEF-5参数升高(P<0.05);能明显改善BPH的主要症状,且起效时间较对照组快(P<0.05或P<0.01);能显著改善和保护性功能,提高IIEF-5得分,明显优于对照组(P<0.05);有改善前列腺质地和减轻疼痛的作用。结论:运用通癃启闭汤治疗BPH患者的疗效可能优于非那雄胺片,其收益为OR=0.22(95%CI=0.04~1.21,95%CI跨过1),NNT=5(95%CI=2.42~81.92),可明显改善BPH患者的生活质量。

Objective ： To observe the effect of Tonglong Qibi decoction on quality of life of patients with benign prostatic hyperplasia （BPH） and the adverse reactions. Methods ： 50 cases of patients with BPH were divided ran- domly into treatment group and control group. The treatment group （n = 25 ） was treated with oral Tonglong Qibi decoction, while the control group （n = 25 ） was treated with Finasteride tablets. The international prostate symp- tom score （IPSS）, international index of erectile function （IIEF -5 ）, quality of life due to dysuria symptoms （QOL）, bladder residual urine volume （PRV） and prostate volume （PV）of patients before and after treatment as well as the complication incidences and side effects of two groups etc. were observed. Results：The total effective rate was respectively 92.00% （95% CI = 73.95% - 99.05% ） in treatment group and 72.00% （95% CI = 54.40% ~ 89.60% ）in control group. Compared the comprehensive effect between two groups（ u = 1. 685 2 ,P = 0. 092 0）, the difference was close to the critical state. Compared with control group, IPSS, QOL and PRV param- eters of treatment group were lowered（ P 〈 0.05 ）, IIEF -5 parameter was raised （P 〈 0.05 ） ;Main symptoms of BPH could be improved significantly, and onset time of action was faster than control group （ P 〈 0.05 or P 〈 0.01 ） ;The sexual function could be improved and protected significantly ,and IIEF -5 score was raised ,which was significantly better than control group （P 〈 0. 05 ） ; Tonglong Qibi decoction had the rolls of improving prostatic texture and relieving pain. Conclusion： The effect of Tonglong Qibi decoction on BPH may be better than that of Finasteride tablets. Its benefit is OR = 0.22 （95% CI =0.04~ 1.21,95% CI crossed 1）, NNT =5 （95% CI =2.42-81.92）. Tonglong Qibi decoction can im- prove the quality of life of patients with BPH.
