多层螺旋CT在结肠癌术前诊断及分期中的应用价值分析

Application value of multi-slice spiral CT in preoperative diagnosis and staging of colon cancer

目的分析多层螺旋CT(MSCT)在结肠癌术前诊断及分期中的应用价值。方法选择2013年8月至2016年8月经我院手术病理确诊为结肠癌患者78例,所有患者术前均接受MSCT扫描检查。以手术病理检查结果为金标准,评估MSCT诊断浆膜外侵犯、淋巴结转移的灵敏度、特异度、准确度,分析结肠癌MSCT特征及分期的准确性。结果患者肠壁呈不规则增厚,或呈不规则形或分叶状肠腔肿块。MSCT诊断浆膜外侵犯与淋巴结转移的灵敏度、特异度、准确度分别为97%、85%、94%;65%、96%、86%。术前MSCT分期结肠癌的总体准确度为95%。结论 MSCT在结肠癌术前诊断与分期中具有显著的应用价值,能够为临床制定合理的手术方案提供指导。

Objective To analyze the application value of multi-sliee spiral CT （MSCT） in preoperative diagnosis and staging of colon cancer. Methods Seventy-eight patients confirmed with colon cancer by operative pathology in our hospital from August 2013 to August 2016 were selected, and all of them were given MSCT scanning. The operative and pathological findings were taken as gold standard. The sensitivity, specificity and accuracy of MSCT in diagnosis of serosal invasion and lymph node metastasis were assessed. The MSCT eharaeteristies of colon cancer and accuracy of staging were analyzed. Results The intestinal wall of patients showed as irregular thickening, or irregular shape and lobulated intraluminal mass.The sensitivity, specificity and aecuracy of MSCT in diagnosis of serosal invasion and lymph node metastasis were 97%, 85%, 94%; 65%, 96%, 86%. The overall accuracy of preoperative MSCT staging of colon cancer was 95%. Conclusion MSCT in preoperative diagnosis and staging of colon cancer has significant application value, which can provide guidance for making reasonable operation plan.
