老年男性病人胰岛素样生长因子-1水平与生活质量及营养状态的相关性分析 被引量:2

The relationship of the insulin-like growth factor-1 levels with quality of life and nutritional status in elderly male patients

摘要 目的:探讨老年男性病人胰岛素样生长因子-1(IGF-1)水平与其生活质量及营养状态的相关性。方法:收集60岁以上老年男性病人50例,分为老年组(60~79岁)20例和高龄组(≥80岁组)30例。2组分别测定血清IGF-1水平,同时采用生活质量健康状况问卷(SF-36)和微量营养评分(MNA-SF)、老年营养风险指数(GNRI)进行生活质量及营养状评价。结果:高龄组病人的血清IGF-1水平为(112.1±46.4)μg/L,低于老年组的(82.8±42.1)μg/L(P<0.05)。除躯体疼痛外,高龄组病人SF-36量表其他7个纬度评分均明显低于老年组(P<0.01);高龄组MNA-SF评分及GNRI亦均明显低于老年组(P<0.01)。相关分析显示,病人IGF-1水平与年龄呈负相关关系(P<0.05)。校正年龄后,病人血清IGF-1水平与SF-36健康量表中生理机能、生理职能、躯体疼痛、活力、总体健康、精神健康、情感职能及营养评分均呈正相关关系(P<0.01),而与社会功能无显著相关性(P>0.05);IGF-1水平与MNA-SF评分及GNRI亦均呈正相关关系(P<0.01和P<0.05)。结论:老年男性病人血清IGF-1水平较低,且与年龄呈负相关关系;血清IGF-1水平的降低影响老年病人生活质量及营养状态。 Objective:To discuss the effects of the changes of insulin-like growth factor-1 ( IGF-1 ) levels on the quality of life and nutritional status in elderly male patients. Methods:A total of 50 elderly male patients (60 -95 years old) were divided into 2 groups according to age : the elderly patient group (60 - 79 years old, n = 20) and the older patient group ( over 80 years old, n = 30). Levels of IGF-1 were detected. The SF-36 questionnaire, micro-nutrient assessment table (MNA-SF), and geriatric nutritional risk index(GNRI) were conducted to evaluate the quality of life and nutritional status. Results: The IGF-1 level was higher in the older patient group [ ( 112.1 ± 46.4)μg/L ] than that in the elderly patient group [ ( 82.8 ± 42.1 ) μg/L ] ( P 〈 0.05 ). In addition to body pain score, scores of other 7 dimensions of SF-36 scale in the older patient group were significantly lower than those in the elderly patient group (P 〈 O. O1 ). Scores of MNA-SF and GNRI in the older patient group were also significantly lower than those in the elderly patient group (P 〈 O. O1 ). Correlation analysis showed that age had a negative correlation with IGF-1 level in elderly male patients(P 〈 0.05). After adjusting the age, IGF-1 level was positively correlated with physical function, role of physical activity, body pain, general health, vitality, mental health, emotional function and nutritional status in SF-36 scale( P 〈 O. 01 ), and had no obvious correlation with social function(P 〉 O. 05). IGF-1 level was also positively correlated with scores of MNA-SF and GNRI(P 〈 0. 01 and P 〈 0.05). Conclusions:The level of serum IGF-1 in elderly male patients was low, and was negatively correlated with age, which can seriously affect the quality of life and nutritional status in elderly patients.
