老年高血压缺血性脑卒中并2型糖尿病患者颈动脉狭窄与血脂的相关性分析

Correlative analysis of carotid artery stenosis and blood lipid in elderly hypertensive patients with ischemic stroke and type 2 diabetes mellitus

目的探讨老年高血压缺血性脑卒中并2型糖尿病患者颈动脉狭窄与血脂的相关性。方法选择该社区卫生服务中心2015年8月‐2017年7月经健康体检后确诊患有缺血性脑卒中,同时合并高血压和2型糖尿病的老年患者共350例作为研究对象,检测患者颈动脉狭窄程度和血脂指标水平,并与健康对照组350例进行比较,观察颈动脉狭窄与血脂(总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、三酰甘油(TG)及高密度脂蛋白胆固醇(HDL-C))的相关性。结果观察组与对照组血浆TG、TC及LDL-C差异有统计学意义(P<0.05);而HDL-C差异无统计学意义(P>0.05);观察组血脂TG、TC及LDL-C水平有狭窄者比无狭窄者高,差异有统计学意义(P<0.05);而HDL-C差异无统计学意义(P>0.05)。结论老年高血压缺血性脑卒中并2型糖尿病患者血脂水平和颈动脉狭窄有密切关系,是引起颈动脉狭窄的危险因素,监测血脂水平及颈动脉斑块对高血压缺血性脑卒中并2型糖尿病高危人群具有重要意义。

【Objective】To investigate the correlation between carotid artery stenosis and blood lipid in elderly patients with essential hypertension and type 2 diabetes mellitus.【Methods】A total of 350 elderly patients diagnosed with ischemic stroke and type 2 diabetes mellitus were included in the observation group, and 350 healthy subjects were included in the control group. Degree of carotid artery stenosis and blood lipid level of the two groups were detected and compared. The correlation between carotid artery stenosis and total cholesterol（TC）, low density lipoprotein cholesterol（LDL-C）, triglyceride（TG） and high density lipoprotein cholesterol（HDL-C） was observed.【Results】There was significant difference in TG, TC and LDL-C between the observation group and the control group（P〈0.05）, but there was no significant difference in HDL-C（P〉0.05）. TG, TC and LDL-C levels in patients with carotid artery stenosis were significant higher than those in patients without carotid artery stenosis in the observation group（P〈0.05）, but there was no significant difference in HDL-C（P〈0.05）.【Conclusion】The blood lipid level in elderly hypertensive patients with ischemic stroke and type 2 diabetes mellitus is closely related to carotid artery stenosis, it＇s a risk factor of carotid artery stenosis, so monitoring blood lipid levels and carotid plaques for high-risk population in hypertensive ischemic brain stroke and type 2 diabetes is of great significance.
