子宫瘢痕妊娠诊治进展

剖宫产术后子宫瘢痕妊娠是一种特殊类型的异位妊娠,现已成为常见的新型妇产科急危重症,超声尤其是经阴道多普勒超声检查、MRI、诊断性宫腔镜检查或腹腔镜检查均有助于本病的诊断,该病一经确诊应及早终止妊娠。方法有药物治疗、介入疗法、宫腔镜及腹腔镜治疗、经阴道或开腹子宫瘢痕妊娠病灶切除加子宫修补术等。瘢痕妊娠的治疗正向个体化、综合化治疗方面快速发展,以期提高疗效、降低创伤,有利于降低子宫瘢痕妊娠对广大孕妇的危害。

Cesarean scar pregnancy( CSP) is a special type of ectopic pregnancy,which had become a common acute and severe disease of obstetrics and gynecology. Ultrasonography especially transvaginal ultrasonography,magnetic resonance imaging( MRI),diagnostic hysteroscopy or laparoscopy are contribute to the diagnosis of CSP, Once CSP is confirmed, immediate termination of pregnancy is warranted, the methods include medical treatment, interventional treatment, hysteroscopic and laparoscopic treatment, excision of the gestational masses by trasnvaginal or open surgery and uterus repair etc. The treatment of CSP is developed towards to be individualized,highly therapeutic,minimally invasive and comprehensive,which could achieve to minimize the risk of the majority of pregnant women with CSP.
