自动心肺复苏机对成人心脏骤停患者影响的前瞻性研究

A Prospective study:Effects of automatic chest compression on adult patients with cardiac arrest

目的探讨自动心肺复苏机对成人心脏骤停患者预后影响。方法前瞻性研究,2014年1月至2016年7月樟树市人民医院急诊收治的成人心脏骤停患者。60例患者入院后随机分为2组,研究组(n=32)采用WFS-01A便携式"自动胸外按压心肺复苏机"进行复苏,对照组(n=28)采用徒手心肺复苏术进行复苏。比较两组患者CPR质量、复苏成功率及相关并发症的影响。结果同对照组相比,机器CPR组的患者平均CPR时间更长[(39.3±13.2)min VS(33.5±11.3)min,P=0.0432)]、CPR中断时间更少[(3.5±1.7)min VS(4.6±2.3)min,P=0.0432,P=0.0380)],但两组患者肋骨骨折发生率无统计学差异(9.38%VS 14.28%,P>0.05)。机器CPR组和徒手CPR组的患者30d生存率(34.76%VS 35.71%,P>0.05)、90d生存率(28.13%VS 25.00%,P>0.05)均无统计学差异。结论采用机械胸外按压方式能减少CPR中断时间,提高心肺复苏质量,但在肋骨骨折发生率,患者30d生存率,90d生存率方面,较徒手心肺复苏无统计学差异。

Objective To discuss the the effect of different external chest compressions on adult patients with cardiac arrest.Methods A prospective study was conducted. adult patients with cardiac arrest admitted to People＇s Hospital of Zhangshu City from Jan.1 st,2014 to Jul.1 st,2017 were enrolled. 60 patients were randomly divided into 2 groups,The study group（n=32） was treated with WFS-01 A portable automatic chest compression cardiopulmonary resuscitation（CPR）,The control group（n=28） was treated with CPR by hand.The quality of CPR,the success rate of resuscitation and the related complications were compared between the two groups. Results Compared with the control group,the patients in the study group had a longer average CPR time[（39.3±13.2）min VS（33.5±11.3）min,P=0.0432）],CPR interrupts were less time [（3.5±1.7）min VS（4.6±2.3）min,P=0.0432,P=0.0380）],However,there was no significant difference in the incidence of rib fracture between the two groups（9.38% VS 14.28%,P〉0.05）. but there was no statistical difference of two groups in the following data,such as 30 d fatality rat（34.76% VS 35.71%,P〉0.05） and 90 d fatality rat（28.13% VS 25.00%,P〉0.05）. Conclusion Using mechanical chest compressions can reduce the CPR interrupt time,improve the quality of CPR,But the two groups patients have no significant difference in the incidence of rib fracture and decreasing mortality.
