摘要
目的:探讨中性粒细胞CD64指数、降钙素原(PCT)和C-反应蛋白(CRP)在血液细菌性感染疾病中的诊断价值。方法:选取2015年1月-2016年12月在本院进行血培养的患者153例,根据血培养结果将其分为血培养阳性组78例和血培养阴性组75例,检测研究对象CD64指数、PCT和CRP水平并进行统计学分析。结果:血培养阳性组与血培养阴性组相比,CD64指数、PCT和CRP水平明显升高(P<0.01)。CD64指数的ROC曲线下面积(AUC)为0.862,PCT的AUC为0.829,而CRP的AUC为0.724,其中CD64指数和PCT的AUC比较,差异无统计学意义(P>0.05),CD64指数和PCT的AUC均明显大于CRP(P<0.001)。CD64指数和PCT诊断血液细菌感染的敏感度、特异度均明显高于CRP,比较差异均有统计学意义(P<0.05)。相关性分析表明,血培养阳性组CD64指数、PCT和CRP水平之间均呈正相关(P<0.05)。结论:CD64指数、PCT和CRP均可有效地预测血液细菌感染。
Objective:To explore the diagnostic value of Neutrophil CD64 index,procalcitonin(PCT) and C-reactive protein(CRP) in blood bacterial infection.Method:153 patients undergoing blood cultures in our hospital from January 2015 to December 2016 were selected as research objects,according to the results of blood culture can be divided into blood culture positive group of 78 cases and blood culture negative group of 75 cases,meanwhile CD64 Index,PCT and CRP were detected for statistical analysis.Result:The CD64 Index、PCT and CRP in blood culture positive group were higher than those of blood culture negative group(P〈0.01).The area under curve(AUC) of CD64 Index was 0.862,the AUC of PCT was 0.829,and the AUC of CRP was 0.724,the AUC of CD64 Index and PCT were compared,the differences was not statistically significant(P〉0.05),the CD64 index and PCT of AUC were obviously larger than those of CRP(P〈0.001).The sensitivity and specificity in diagnosing blood bacterial infection of CD64 Index and PCT were higher than those of CRP,and the difference was statistically significant(P〈0.05).Correlation analysis showed that there were positive correlation between CD64 Index,PCT and CRP level in blood culture positive group(P〈0.05).Conclusion:CD64 Index,PCT and CRPcan effectively predict blood bacterial infections.
